摘  要:目的探讨Lenstar LS900光学生物测量与浸润式B型超声引导下的A型超声分段式生物测量法在高度近视眼眼轴长度测量中的一致性。方法收集2018年1月至2019年9月就诊于浙江省杭州西湖朝聚眼科医院白内障科的白内障伴高度近视眼患者36例(65只眼)的病例资料。其中,男性21例(41只眼),女性15例(24只眼);年龄50~81岁,平均年龄(63±2.1)岁;等效球镜屈光度为-11.50^-10.50 D,平均等效球镜屈光度为(-11.00±0.50)D。分别应用Lenstar LS900光学生物测量和A/B型超声诊断仪采用浸润式B型超声引导下的A型超声分段式生物测量法检测高度近视眼眼轴的长度。采用配对t检验比较两检测方法测得眼轴的长度,采用Pearson相关分析评价其相关性,采用Bland-Altamn统计分析评价其一致性。结果 Lenstar LS900光学生物测量和浸润式B型超声引导下的A型超声分段式生物测量法测量患者眼轴的平均长度分别为(29.01±2.04)mm和(28.91±2.08)mm,二者差异无统计学意义(t=-0.645,P>0.05)。两种检测方法获得的眼轴长度差值的平均值为(0.11±0.09)mm,95%一致性界限区间为-1.42^+1.71。结论 Lenstar LS900光学生物测量与浸润式B型超声引导下的A型超声分段式生物测量法检测高度近视眼眼轴长度有着相同的准确性,后者为实时二维图像,可准确引导A型超声测量至黄斑中心位点,避免后巩膜葡萄肿及其它眼底病变的干扰,具有良好的重复性和可靠性。Objective The aim of this study was to compare the accuracy and consistency between Lenstar LS900 optical biometry and A-scan segment biometric measurement guided by infiltrating B ultrasound in axial length of high myopia. Methods 36 patients(65 eyes) with cataract and high myopia were treated in the Department of Cataract, Hangzhou West Lake Chaoju Ophthalmic Hospital of Zhejiang Province from January 2018 to September 2019 were selected. Among of them, there were 21 males(41 eyes) and 15 females(24 eyes), age(50 to 81) with mean age(63±2.1) years-old;spherical mirror-11.50^-10.50 D, mean equivalent spherical mirror(-11.00±0.50) D. Lenstar LS900 optical biometric instrument and A/B scanner ophthalmic ultrasound guided by infiltrating B ultrasound were used to measure the biometric data, respectively. The differences between results were compared by paired t test. The correlation of the measured data was analyzed by Pearson correlation analysis, and the consistency was measured by Bland-Altamn statistical analysis. Results The average length of patients using Lenstar LS900 optical biometry was(29.01±2.04) mm, and the average length of the eye axis of patients using A-scan segment biometric measurement guided by infiltrating B ultrasound was(28.91±2.08) mm. And there was no statistically significant difference between two devices(t=-0.645, P>0.05). The average value of the difference in axial length obtained using two devices was( 0. 11 ± 0. 09) mm;95% consistency interval was-1. 42 to + 1. 71. Conclusions Lenstar LS900 optical biological has the same accuracy as the A-scan segment biometric measurement guided by infiltrating B ultrasound. The latter is a real-time with twodimensional image,which can accurately guide A-ultrasound measurement to the central site of the macula to avoid the interference of posterior scleral staphyloma and other fundus diseases with good repeatability and reliability.
