单环套扎器联合双臂钳修补消化道穿孔1例

摘　　要：胃肠道间质瘤(GIST)是间叶源性肿瘤,过去的首选治疗方式是外科手术切除。目前,随着内镜技术的不断发展,内镜全层切除术(EFTR)在GIST等黏膜下肿瘤的治疗中取得了较好的效果,但完整剥离肿物及包膜将不可避免地出现主动性穿孔。本例患者术中出现主动性穿孔,并行内镜下单环套扎器联合双臂钳成功封闭穿孔。单环套扎器联合双臂钳修补穿孔具有良好的效果,可节约手术时间,减少并发症的发生及手术费用,有较高的临床应用价值。Gastrointestinal stromal tumors(GIST)are mesenchymal tumors.In the past,surgical resection was the preferred treatment method.At present,with the continuous development of endoscopic techniques,endoscopic full-thickness resection(EFTR)has achieved good effect in the treatment of submucosal tumors,such as GIST,but active perforation will inevitably occur if the tumor and capsule are completely peeled off.In this case,active perforation occurred during the operation,and the perforation was closed successfully with single-loop ligation combining with double-arm forceps under endoscopy.Repairing perforation with single-loop ligation combining with double-arm forceps has good effect,can save operation time,reduce the occurrence of complications and operation cost,and has high clinical application value.
