摘 要:目的探讨盐酸米诺环素联合牙周基础治疗对中重度牙周炎患者血清炎性细胞因子水平的影响。方法选择2016年8月至2017年8月于新乡医学院第三附属医院口腔科就诊的80例中重度牙周炎患者为研究对象,按照随机数字表法将所有患者分为对照组和观察组,每组40例。对照组患者给予牙周基础治疗,观察组患者在牙周基础治疗的基础上给予盐酸米诺环素凝胶治疗。比较2组患者治疗前、治疗后3个月牙周探诊深度(PD)、临床附着丧失(CAL)、龈沟出血指数(SBI)及血清肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-1β、IL-6及C反应蛋白(CRP)水平。结果2组患者治疗前PD、CAL、SBI比较差异无统计学意义(P>0.05)。治疗后3个月,2组患者PD、CAL、SBI均显著低于治疗前(P<0.01);观察组患者PD、CAL、SBI显著低于对照组(P<0.01)。2组患者治疗前血清TNF-α、IL-1β、IL-6、CRP水平比较差异无统计学意义(P>0.05)。治疗后3个月,2组患者血清TNF-α、IL-1β、IL-6、CRP水平显著低于治疗前(P<0.01);观察组患者血清TNF-α、IL-1β、IL-6、CRP水平显著低于对照组(P<0.01)。结论盐酸米诺环素凝胶联合牙周基础治疗能够显著改善牙周炎患者的牙周状况,降低血清炎性细胞因子水平。Objective To explore the effect of minocycline hydrochloride combined with periodontal therapy on the level of serum inflammatory cytokines in patients with moderate and severe periodontitis.Methods From August 2016 to August 2017,eighty patients with moderate-severe periodontitis treated in the Department of Stomatology,the Third Affiliated Hospital of Xinxiang Medical University were selected as the study subjects.All patients were divided into control group and observation group according to the random number table method,with 40 cases in each group.The patients in the control group were treated with periodontal basic therapy.Based on the therapy of the control group,the patients in the observation group were treated with minocycline hydrochloride.The periodontal probing depth(PD),clinical attachment loss(CAL),sulcus bleeding index(SBI)and the serum levels of tumor necrosis factor-α(TNF-α),interleukin(IL)-1β,IL-6 and C-reactive protein(CRP)were compared before treatment and at three months after treatment.Results There was no significant difference in the PD,CAL and SBI between the two groups before treatment(P>0.05).At three months after treatment,the PD,CAL and SBI in the two groups were lower than those in the observation group(P<0.01);and the PD,CAL and SBI in the observation group were lower than those in the control group(P<0.01).There was no significant difference in the levels of TNF-α,IL-1β,IL-6 and CRP between the two groups before treatment(P>0.05).At three months after treatment,the levels of TNF-α,IL-1β,IL-6 and CRP in the two groups were significantly lower than those before treatment(P<0.01);and the levels of TNF-α,IL-1β,IL-6 and CRP in the observation group were significantly lower than those in the control group(P<0.01).Conclusion Minocycline hydrochloride combined with periodontal basic therapy can significantly improve the periodontal condition of patients with periodontitis and reduce the level of serum inflammatory cytokines.
