三间隙引流术对肛周脓肿患者疼痛及创面愈合的影响

摘　　要：目的:探讨三间隙引流术对肛周脓肿患者疼痛及创面愈合的影响。方法:选取2014年1月~2018年1月于我院肛肠科收治的100例肛周脓肿患者为研究对象,按随机数字表法分为对照组(n=50)和观察组(n=50)。对照组用肛周脓肿切开引流术,观察组用三间隙引流术。比较两组患者术后创口红肿、渗液评分、创面面积、愈合情况、肛门疼痛程度、外周血白细胞水平、术后并发症和患者满意度。结果:与对照组相比,观察组患者术后1周创口红肿、渗液评分均明显降低,创面面积明显缩小,且愈合时间明显缩短(均P<0.01)。两组患者术后首次排便后VAS评分无统计学意义(P>0.05),但观察组患者术后3 d及5 d排便后的VAS评分显著低于对照组,差异均有统计学意义(均P<0.01)。观察组术后2 w外周血白细胞水平低于对照组,肛门功能恢复率及患者满意度均高于对照组,差异均有统计学意义(均P<0.01)。结论:三间隙引流术治疗肛周脓肿,可促进创口愈合,降低并发症的发生率和疼痛程度,同时对肛门功能影响较小,满意度较高,值得临床推广。Objective:To explore the effect of three-gap drainage on pain and wound healing in patients with perianal abscess.Methods:A total of 100 patients with perianal abscess from January 2014 to January 2018 were randomly divided into the control group(n=50)and the observation group(n=50)according to random number table.The control group was treated with perianal abscess incision and drainage,while the observation group was treated with three-gap drainage.The postoperative swelling,osmotic score,wound area,wound healing,anal pain,peripheral blood leukocyte level,complications and patient satisfaction were recorded and compared between the two groups.Results:Compared with the control group,the scores of swelling and exudation,as well as the wound area were all significantly reduced in the observation group one week after operation(all P<0.01).What's more,the healing time was also significantly shortened in the observation group than that of control group(P<0.01).There was no significant difference in VAS score between two groups after the first defecation(P>0.05).However,the VAS score at 3 and 5 days of postoperative defecation in the observation group was significantly lower than that of control group(both P<0.01).The level of peripheral blood white blood cells at 2 weeks after operation was lower,while the recovery rate of anal function and the patients'satisfaction were higher in the observation group when compared to the control group(all P<0.01).Conclusion:In patients with perianal abscess,three-gap drainage can promote wound healing,and reduce the incidence of complications and pain,with little impact on anal function and better satisfaction,which is worthy of further clinical promotion.
