摘  要:目的探讨出血性脑卒中患者凝集素样氧化型低密度脂蛋白受体-1的水平变化及临床意义。方法选取2017年1月-2018年6月本院收治的250例脑卒中患者作为研究对象,根据其类型分为出血性脑卒中组(n=100)和缺血性脑卒中组(n=150);比较2组患者的血清凝集素样氧化型低密度脂蛋白受体-1的水平,并依据出血量和神经功能损伤程度对出血性脑卒中患者进行分组,比较不同组别患者血清凝集素样氧化型低密度脂蛋白受体-1的表达水平;采用logistic回归方法分析凝集素样氧化型低密度脂蛋白受体-1水平与出血性脑卒中的相关性。结果出血性脑卒中组sLOX-1水平(1.56±0.23 ng/mL)显著高于缺血性脑卒中组(0.48±0.11 ng/mL)(P<0.05);不同出血量和神经功能损伤程度的患者均在7 d内sLOX-1水平逐渐升高,且第7 d达到峰值,随后逐渐下降(P<0.05);大量出血组相同时间点sLOX-1水平高于小出血量及中出血量组(P<0.05);重度组相同时间点sLOX-1水平高于轻度及中度组(P<0.05);Logistic回归分析显示sLOX-1水平上升可以增加出血性脑卒中的发病风险(OR=1.943,95%CI=1.642~2.238,P=0.000)。结论出血性脑卒中患者的血清凝集素样氧化型低密度脂蛋白受体-1水平显著增高,且与脑出血量及神经功能损伤程度有关,有望作为出血性脑卒中的生物标记物。Objective To investigate the level change and clinical significance of lectin-like oxidized low density lipoprotein receptor-1(LDL-1) in patients with hemorrhagic stroke. Methods From January 2017 to June 2018, 250 patients with cerebral apoplexy were divided into hemorrhagic stroke group(n=100) and ischemic stroke group(n=150) according to the type of stroke. The levels of serum lectin-like oxidized low density lipoprotein receptor-1 were compared between the two groups, and the amount of hemorrhage and the degree of neurological damage were compared. The patients in hemorrhagic stroke group were divided into two groups, and the levels of serum lectin-like oxidized low density lipoprotein receptor-1 in different groups were compared.Logistic regression was used to analyze the correlation between the level of lectin-like oxidized low density lipoprotein receptor-1 and hemorrhagic stroke. Results The level of sLOX-1 in hemorrhagic stroke group [(1.56±0.23) ng/mL] was significantly higher than that in ischemic stroke group [(0.480.11) ng/mL ](P<0.05). The level of sLOX-1 in the patients with different bleeding volume and different degree of nerve function injury increased gradually within 7 days, and reached the maximum value on the 7 th day, then decreased gradually(P<0.05). On the same day, the level of sLOX-1 in the large hemorrhage group was higher than that in the small and moderate hemorrhage group(P<0.05), and the sLOX-1 level in the severe injury group was higher than that in the mild and moderate injury group(P<0.05). Logistic regression analysis showed that increased level of sLOX-1 increased the risk of hemorrhagic stroke(OR=1.943,95%CI=1.642~2.238,P=0.000). Conclusion The level of lectin-like oxidized low density lipoprotein receptor-1 in patients with hemorrhagic stroke was significantly higher than that in patients with ischemic stroke, and it was associated with the amount of cerebral hemorrhage and nerve function injury degree. It was expected to be a biomarker of hemorrhagic stroke.
