腹腔镜结肠癌根治术治疗T4a期非转移性结肠癌对术后感染及炎性因子的影响

摘　　要：目的:探讨腹腔镜结肠癌根治术治疗T4a期非转移性结肠癌对术后感染及炎性因子的影响。方法:选择笔者所在医院2016年1月-2019年1月收治的72例T4a期非转移性结肠癌患者,采用随机数字表法分为腹腔镜组与开腹组,各36例,分别给予腹腔镜结肠癌根治术与开腹结肠癌根治术。比较两组手术相关指标、感染情况及手术前后血清炎性因子水平。结果:腹腔镜组术中出血量少于开腹组,术后排气时间、术后住院时间均短于开腹组,总感染率低于开腹组,术后1周CRP、TNF-α、IL-6水平均低于开腹组,差异均有统计学意义(P<0.05)。结论:腹腔镜结肠癌根治术治疗T4a期非转移性结肠癌安全可行,可减少术后感染,能明显改善患者炎性反应。Objective:To explore the effect of laparoscopic radical resection of colon cancer on postoperative infection and inflammatory cytokines of nonmetastatic colon cancer of T4a stage.Method:A total of 72 patients with non-metastatic colon cancer of T4a stage in our hospital from January 2016 to January 2019 were selected.Patients were randomly divided into the laparoscopic group and the open group,with 36 cases in each group,and patients were given laparoscopic radical resection of colon cancer and open radical resection of colon cancer respectively.The operation related indicators,infection situation and levels of serum inflammatory cytokines before and after operation were compared between the two groups.Result:The intraoperative blood loss in the laparoscopic group was less than that of the open group,postoperative exhaust time and postoperative hospital stay were shorter than those of the open group,total infection rate was lower than that of the open group,and CRP,TNF-αand IL-6 levels one week after the operation were lower than those of the open group,and the differences were statistically significant(P<0.05).Conclusion:For patients with non-metastatic colon cancer of T4a stage,laparoscopic radical resection of colon cancer is safe and feasible,it can reduce postoperative infection and significantly improve patients’inflammatory response.
