摘 要:目的探讨肝癌脑转移瘤CT及MR影像表现。方法回顾性分析该院自2016年5月—2018年3月期间经手术病理证实的8例肝癌脑转移瘤的影像学资料。结果该组8例肝癌脑转移中,7例为单发,1例多发,7例发生于幕上,1例大小脑均有转移灶,其中1例累及颅骨及颞骨、脑外皮下软组织。CT为高密度,MRT1为高或高低混杂信号,增强后扫描病灶实质环状或结节状明显强化,出血灶不强化。MR增强扫描显示病灶范围明显优于CT。结论肝癌发生脑转移瘤的影像表现,为多单发,易出血,MR增强检查为最佳的影像检查技术。Objective To investigate the CT and MR imaging features of brain metastases from hepatocellular carcinoma.Methods The imaging data of 8 cases of hepatocellular carcinoma with brain metastases confirmed by operation and pathology in the hospital from May 2016 to March 2018 were retrospectively analyzed.Results Among the 8 cases of brain metastasis of hepatocellular carcinoma,7 were single,1 multiple,7 were supratentorial,1 had metastases in both cerebellum and cerebellum,and 1 involved both the humerus and extrahepatic subsutaneous soft tissue.4 cases were complicated with cerebral hemorrhage,irregular shape,high density on CT,high or low mixed signal on MRT1.After contrast enhancement,the solid ring or nodule of the lesion was obviously enhanced,but the hemorrhagic focus was not enhanced.Enhanced MR scan showed that the lesion size was significantly better than that of CT.Conclusion The imaging manifestations of hepatocellular carcinoma with brain metastases are multiple,single and prone to bleeding.MR enhancement is the best imaging technique.
