摘  要:目的探讨基于计算机辅助设计(computer-aided design,CAD)与3D打印技术的血管化腓骨游离移植修复下颌骨缺损的应用效果。方法选择22例需行下颌骨病变切除及重建的患者,并随机分为A、B两组,A组采用CAD与3D打印技术的血管化腓骨游离移植修复下颌骨缺损,B组采用传统血管化腓骨游离移植修复下颌骨缺损,记录手术时间,将术前术后三维图像重叠,分别测量下颌骨各标志点的移动距离,并将两组结果进行统计分析。结果A组和B组平均手术时间分别为(8.28±1.17)h和(9.45±0.94)h,差异有统计学意义(P=0.018)。A组下颌颏顶点的移动距离较B组下颌颏顶点的移动距离短,A组各位置点的总体移动距离较B组各位置点的总体移动距离短,且差异均有统计学意义,其余位置点平均移动距离未见明显差异。结论CAD与3D打印技术在血管化腓骨游离移植修复下颌骨缺损方面,可以减少手术时间,较为理想的恢复颌面外形,获得较高的精确度。Objective To investigate the application effect of vascularized fibular free grafting to repair mandibular defects based on computer-aided design(CAD)and 3 D printing technology.Methods A total of 22 patients requiring resection and reconstruction of mandibular lesions were randomly divided into two groups.Patients of group A were treated with CAD and 3 D printing techniques for vascularized fibular free grafting to repair mandibular defects.And patients of Group B were treated with conventional vascularized fibular free transplantation.The operation time was recorded.The three-dimensional images of preoperative and postoperative were superimposed.The moving distances of the mandibular landmarks were measured,and the results of the two groups were statistically analyzed.Results The mean operative time in group A and group B was(8.28±1.17)h and(9.45±0.94)h,respectively(P=0.018),the difference between the two groups was statistically significant.The moving distance of mandibular chin apex in group A was shorter than that in group B.In addition,the overall moving distance of each point in group A was shorter than that in group B,and the difference was statistically significant.There was no significant difference in the average moving distance of other points.Conclusion CAD and 3 D printing technology can reduce the operation time in the treatment of mandibular defects with vascularized fibular free grafting,and it is ideal to restore the maxillofacial shape and obtain higher precision.
