摘  要:目的分析研究糖类抗原CA199、糖类抗原CA125以及癌胚抗原CEA在肝细胞癌诊断中的应用价值。方法对该院2017年1月—2019年1月接诊的124例肝细胞癌患者病历资料(CA199、CA125以及CEA检验结果)进行回顾性分析,并随机抽取同时期体检并化验CA199、CA125以及CEA的124名健康体检者资料作为对比,对比两组患者血清CA199、CA125以及CEA水平以及CA199、CA125以及CEA单独与联合检测在肝细胞癌患者中的的阳性率。结果肝细胞癌患者的CA199、CA125以及CEA检验值均显著高于健康体检者,差异有统计学意义(t=20.762、24.121、19.638,P<0.05),3种检测方法单独检测在肝细胞癌患者中的阳性率分别为48.39%(60/124)、33.87%(42/124)以及65.32%(81/124),显著低于3种检测方法联合检测的阳性率94.35%(117/124),差异有统计学意义(χ^2=19.766、20.085、28.731,P<0.05);3种检测方法单独检测在健康体检者中的阳性率分别为12.10%(15/124)、14.52%(18/124)及13.71%(17/124),与3种方法联合检测阳性率16.94%(21/124),差异无统计学意义(χ^2=1.174、1.033、1.247,P>0.05),肝细胞癌患者3种方法单独检测及联合检测阳性率均显著高于健康体检者,差异有统计学意义(χ^2=12.467、10.586、20.752、38.531,P<0.05)。结论CA199、CA125以及CEA在肝细胞癌诊断中均具有较高的临床应用价值,3种检测联合应用,可显著提高对肝细胞癌的诊断准确率,应在临床上广泛推广应用。Objective To study the application value of carbohydrate antigen CA199,carbohydrate antigen CA125 and carcinoembryonic antigen CEA in the diagnosis of hepatocellular carcinoma.Methods The medical records of 124 patients with hepatocellular carcinoma(CA199,CA125 and CEA test results)from January 2017 to January 2019 in the hospital were retrospectively analyzed,and the same physical examination and CA199,CA125 were performed.The data of 124 healthy subjects in CEA were compared.The positive rates of serum CA199,CA125 and CEA levels and CA199,CA125 and CEA alone and in combination were detected in patients with hepatocellular carcinoma.Results The CA199,CA125 and CEA values of patients with hepatocellular carcinoma were significantly higher than those of healthy subjects.The difference was statistically significant(t=20.762,24.121,19.638,P<0.05).Three detection methods were detected in the liver alone.The positive rates in patients with cell carcinoma were 48.39%(60/124),33.87%(42/124),and 65.32%(81/124),respectively,which was significantly lower than the combined detection rate of 94.35%(117/124),the difference was statistically significant(χ^2=19.766,20.085,28.731,P<0.05);the positive rates of the three detection methods in healthy subjects were 12.10%(15/124)and 14.52%(18/124)and 13.71%(17/124),the positive rate of combined detection with three methods was 16.94%(21/124),and the difference was not statistically significant(χ^2=1.174,1.033,1.247,P>0.05).The positive rates of the three methods of detection and combined detection were significantly higher in patients with hepatocellular carcinoma than those in healthy subjects,and the difference was statistically significant(χ^2=12.467,10.586,20.752,38.531,P<0.05).Conclusion CA199,CA125 and CEA have high clinical value in the diagnosis of hepatocellular carcinoma.The combination of three tests can significantly improve the diagnostic accuracy of hepatocellular carcinoma and should be widely applied in clinic.
