摘　　要：目的探讨在永久性结肠造口患者的全程护理中实施术前定位与康复期锻炼对造口旁疝的预防控制作用。方法研究对象为结肠直肠癌中心2016年1月—2017年7月105例行永久性结肠造口的患者,按照随机数字表法分为两组,52例为对照组,在造口全程护理基础上实施术前定位与常规干预;53例为观察组,在造口全程护理基础上实施术前定位与康复期锻炼,比较两组效果。结果观察组术后2年内造口旁疝发生率为5.66%,低于对照组术后2年内造口旁疝发生率19.23%(χ^2=4.456,P<0.05);观察组干预后社会交往、心理状态、生理功能方面的适应性评分均高于对照组(P<0.05);观察组干预后造口知识掌握度评分为(49.89±7.48)分,高于对照组(44.70±6.85)分(t=3.706,P<0.05)。结论在永久性造口患者的护理中开展术前定位以及康复期锻炼能够有效减少造口旁疝的发生,提升患者对造口的适应性,增加患者对健康知识的掌握。Objective To investigate the preventive and preventive effects of preoperative positioning and rehabilitation exercise on parastomal hernia in patients undergoing permanent colostomy.Methods The subjects who underwent a permanent colostomy in the colorectal cancer center from January 2016 to July 2017 were randomly divided into 2 groups and 52 patients in the control group,preoperative positioning and routine intervention were performed on the basis of nursing;53 cases in the observation group,and the preoperative positioning and rehabilitation period were performed on the basis of the whole process of stoma,and the effects of the two groups were compared.Results The incidence of parastomal hernia in the observation group was 5.66% within 2 years after operation.The incidence of parastomal hernia was 19.23%(χ^2=4.456,P<0.05)in the 2 years after operation.The observation group received social interaction after intervention.The adaptive scores of mental state and physiological function were higher than those of the control group(P<0.05).The score of ostomy knowledge in the observation group was(49.89±7.48)points,which was higher than that of the control group(44.70±6.85)points(t=3.706,P<0.05).Conclusion Preoperative positioning and rehabilitation during the care of permanent stoma patients can effectively reduce the occurrence of parastomal hernia,improve the patient's adaptability to the stoma,and increase the patient's mastery of health knowledge.
