探讨延伸护理对永久性结肠造口患者生活质量的影响

摘　　要：目的探讨居家延伸护理对永久性结肠造口生活质量的影响效果。方法将该院于2016年3月—2017年7月收治的85例永久性结肠造口患者随机分为两组,对照组42例,接受出院后的常规电话随访,观察组43例,除了接受出院后的常规电话随访外,再通过延伸护理小组实施居家延伸护理,比较两组半年后的自护能力、生活质量。结果观察组干预半年后自我概念、健康知识水平、自我护理技能、自护责任感评分为(25.48±4.99)分、(67.54±6.80)分、(39.45±6.38)分、(20.45±4.28)分,均高于对照组(21.38±3.66)分、(55.72±5.94)分、(32.45±6.29)分、(17.55±3.81)分,差异有统计学意义(t=4.268、3.854、4.215、4.849,P<0.05);观察组干预半年后一般健康状况、精力、精神健康、生理机能、躯体疼痛、生理功能、情感职能、社会功能评分均高于对照组,差异有统计学意义(P<0.05)。结论对永久性结肠造口患者在出院后实施居家延伸护理可明显提升患者自护能力,改善患者出院后的生活质量,有良好的临床推广应用价值。Objective To explore the effect of extended home care on the quality of permanent colostomy.Methods Eighty-five patients with permanent colostomy from March 2016 to July 2017 were randomly divided into two groups,42 patients in the control group,followed by regular telephone follow-up after discharge,and 43 patients in the observation group,except for the routine after discharge.After telephone follow-up,the extended care team was used to implement home extension care,and the self-care ability and quality of life of the two groups after half a year were compared.Results After six months of intervention,the self-concept,health knowledge level,self-care skills,and self-care responsibility scores were(25.48±4.99)points,(67.54±6.80)points,(39.45±6.38)points,and(20.45±4.28)points,were higher than the control group(21.38±3.66)points,(55.72±5.94)points,(32.45±6.29)points,and(17.55±3.81)points,The difference was statistically significant(t=4.268,3.854,4.215,4.849,P<0.05).the general health status,energy,mental health,physiology,physical pain,physiological function,emotional function,and social function scores of the observation group were higher than those of the control group,The difference was statistically significant(P<0.05).Conclusion The implementation of home extension care after discharge from patients with permanent colostomy can significantly improve the patient's self-care ability,improve the quality of life of patients after discharge,and have good clinical application value.
