摘　　要：目的将现有的促进NICU住院早产儿母亲泵乳的最佳证据应用到临床实践中。方法采用JBI循证卫生保健中心的临床证据实践应用系统,基于现有的最佳证据,制定7条审查标准,于2018年7-10月对146名医护人员和30名NICU住院早产儿母亲及其配偶进行基线审查和证据应用后的再审查。结果证据应用后,除第1条审查标准在基线审查时已达100%,其余审查标准的达标率均显著上升(P<0.05);医护人员、早产儿母亲和父亲的母乳喂养和早期泵乳相关知识得分均显著提高(P<0.01);早产儿母亲48 h内的泵乳次数和泌乳量均显著增加(P<0.01)。结论基于循证的质量审查项目可改善NICU住院早产儿母亲早期泵乳的临床实践,但今后仍需要长期持续的质量审查,以不断促进早产儿的母乳喂养实践。Objective To promote early breast milk expression among mothers of preterm infants in NICU.Methods The JBI Practical Application of Clinical Evidence System(PACES) and Getting Research into Practice(GRiP) audit and feedback tool were used in this project. Based on the available best evidence, sevenaudit criteria were developed, and 146 health care professionals and 30 mothers of preterm infants in NICU, as well as their spouses were surveyed in the baseline audit and follow-up audit.Results The first audit criterion had already reached 100% in the baseline audit. An overall significant improvement was observed for the other audit criteria after the implementation of various strategies(P<0.05). The scores of breast-feeding and early breast milk expression related knowledge of health care professionals, mothers and their spouses were significantly increased(P<0.01). The number of breast milk expression and volume of breast milk production within 48 hours were significantly increased(P<0.01). Conclusions The implementation of evidence-based practice was effective in promotion of early breast milk expression among mothers of preterm infants in NICU. Future audits may be undertaken to evaluate the long-term effect of the implementation.
