血清相关标志物CEA、CA50、CA19-9对胃癌的诊断价值

摘　　要：目的探讨血清相关标志物癌胚抗原(carcinoembryonic antigen,CEA)、糖蛋白抗原(carbohydrate antigen,CA) 50、CA19-9对胃癌的诊断价值。方法选择手术切除并经病理证实的胃癌患者123例作为胃癌组,同期选择胃炎患者224例作为胃炎组。检测2组血清相关标志物CEA、CA50、CA19-9含量并判断阳性率,判定诊断价值。结果胃癌组的血清CEA、CA50、CA19-9含量都显著高于胃炎组(P <0. 05)。胃癌组的血清CEA、CA50、CA19-9阳性率分别为82. 9%、79. 7%和82. 9%,都显著高于胃炎组的20. 1%、14. 7%和27. 7%(P <0. 05)。在347例患者中,Spearman等级相关分析显示CEA、CA50、CA19-9阳性率与胃癌存在显著正相关性(P <0. 05)。ROC曲线分析显示CEA、CA50、CA19-9诊断胃癌曲线下面积A值分别为0. 669、0. 602和0. 728。结论血清相关标志物CEA、CA50、CA19-9在胃癌患者中呈现高表达状况,与胃癌的发生显著相关,可用来早期鉴别诊断胃癌。Objective To investigate the diagnostic values of serum-related markers carcinoembryonic antigen( CEA),carbohydrate antigen( CA) 50,and CA19-9 in gastric cancer. Methods 123 patients with gastric cancer who underwent surgical resection and pathological confirmation were selected as the gastric cancer group,and 224 patients with gastritis were selected as the gastritis group. The serum related markers CEA,CA50,CA19-9 were detected and the positive rates were determined,and were to determine the diagnostic values. Results Serum CEA,CA50 and CA19-9 levels in the gastric cancer group were significantly higher than those of the gastritis group( P < 0. 05). The positive rates of serum CEA,CA50 and CA19-9 in the gastric cancer group were 82. 9%,79. 7% and 82. 9%,respectively,which were significantly higher than those in the gastritis group( 20. 1%,14. 7% and 27. 7%)( P < 0. 05). In 347 patients,Spearman rank correlation analysis showed that CEA,CA50,CA19-9 positive rate was significantly positively correlated with gastric cancer( P < 0. 05). ROC curve analysis showed that the area A under the curve of CEA,CA50,CA19-9 diagnosis of gastric cancer was 0. 669,0. 602 and 0. 728,respectively. Conclusion The serum related markers CEA,CA50,CA19-9 have high expression in gastric cancer patients,which are significantly correlated with the occurrence of gastric cancer,and can be used for early differential diagnosis of gastric cancer.
