摘 要:目的探讨胰岛素受体底物1(IRS-1)与胰岛素受体底物2(IRS-2)在乳腺癌组织中的表达与患者临床病理特征和预后的关系。方法选取70例乳腺癌患者作为研究对象,采用系统回顾性分析法分析所有患者的临床资料,根据资料结果可分为观察组(70份癌组织标本)及对照组(70份癌旁组织标本,距肿瘤边缘4 cm处),并采用免疫组织化学法检测IRS-1、IRS-2蛋白的表达,比较IRS-1、IRS-2蛋白表达与乳腺癌患者临床病理特征以及预后的关系。结果乳腺癌组织中IRS-1、IRS-2蛋白的高表达率明显均高于对照组;IRS-1、IRS-2蛋白表达与乳腺癌患者年龄、肿瘤大小、组织学分级以及雌激素受体状况无明显相关(P> 0. 05),但IRS-1、IRS-2蛋白表达与乳腺癌患者的TNM分期以及淋巴结转移有明显相关(P <0. 05);经单因素分析得出:TNMⅢ~Ⅳ期、有淋巴结转移以及IRS-1、IRS-2高表达患者生存时间均显著缩短(P <0. 05);经多因素Cox比例风险回归模型分析得出:淋巴转移、IRS-1高表达、IRS-2高表达以及TNMⅢ~Ⅳ期均为影响乳腺癌患者预后的独立危险因素(P <0. 05)。结论 IRS-1、IRS-2在乳腺癌组织中的表达水平增高,其表达水平与乳腺癌患者的淋巴结转移、TNM分期均有显著相关,与乳腺癌的发生和发展有重要意义。Objective To investigate the expression of insulin receptor substrate 1( RS-1) and insulin receptor substrate2( IRS-2) in breast cancer tissues and their relationship with clinicopathological features and prognosis. Methods 70 patients with breast cancer were selected as subjects,and their clinical data were analyzed through systematic retrospective analysis,They were divided into the observation group( 70 cancer tissue specimens) and the control group( 70 adjacent tissue specimens at 4 cm from the tumour edge). The expression of IRS-1 and IRS-2 protein was detected by immunohistochemical method,and the relationship with clinicopathological features and prognosis of breast cancer was analyzed. Results The high expression rates of IRS-1 and IRS-2 proteins in breast cancer tissues were significantly higher than those of the control group. There was no obvious correlation between the expression of IRS-1 and IRS-2 proteins and age,tumor size,histological grade or status of estrogen receptor in breast cancer patients( P > 0. 05),while the expression of IRS-1 and IRS-2 proteins was significantly correlated with TNM staging and lymph node metastasis of breast cancer( P < 0. 05). Univariate analysis showed that the survival time of patients at TNM stage III ~ IV,patients with lymph node metastasis and high expression of IRS-1 and IRS-2 was significantly shorter( P < 0. 05). Multivariate Cox proportional hazards regression model analysis showed that lymph metastasis,high expression of IRS-1 and IRS-2,and TNM III ~ IV were independent risk factors for the prognosis of breast cancer patients( P < 0. 05). Conclusion The expression levels of IRS-1 and IRS-2 are increased in breast cancer tissues and they are significantly related to lymph node metastasis and TNM stage. They are important for the occurrence and development of breast cancer.
