消癌平联合吉非替尼对进展期非小细胞肺癌患者凝血功能及临床疗效的影响

摘　　要：目的探讨消癌平注射液联合吉非替尼治疗进展期非小细胞肺癌(NSCLC)的临床疗效及对患者凝血功能的影响。方法将173例ⅢA~Ⅳ期NSCLC患者随机分为对照组(n=77)和观察组(n=96)。对照组患者接受吉非替尼治疗,观察组患者接受消癌平注射液联合吉非替尼治疗。比较2组患者治疗前后血流动力学指标、凝血分子水平变化以及临床治疗效果。结果治疗后,观察组患者治疗总有效率为92.71%,明显高于对照组(P<0.05)。对照组患者有49例(63.64%)出现高凝状态,明显高于治疗前和观察组(P<0.05)。经治疗后,观察组患者血清纤维蛋白原和D-二聚体含量、全血粘度高切值低于治疗前和对照组(P<0.05);而全血粘度低切值、毛细管血粘度值、红细胞压积和红细胞聚集指数均低于对照组(P<0.05)。结论消癌平注射液可以改善进展期非小细胞肺癌患者的血液高凝、高粘状态,联合吉非替尼抗癌效果理想,值得临床推广。Objective To discuss the clinical effect and the influence of Xiao Ai Ping injection combined with gefitinib on the blood coagulation state of patients with advanced non-small cell lung cancer(NSCLC).Methods 173 patients with stageⅢA~ⅣNSCLC were randomly divided into the control group(n=77)and the observation group(n=96).The patients in the control group were treated with gefitinib;and the patients in the observation group were treated with Xiao Ai Ping injection combined with gefitinib.The clinical effect and the serum levels of blood coagulation related parameters were compared between the 2 groups.Results After treatment for 3 m,the total efficacy rate of patients in the observation group was 92.71%,which was higher than the control group(P<0.05).And 49 patients(63.64%)with high blood coagulation state in the control group,which was higher than before treatment and the observation group(P<0.05).Besides,the serum values of fibrinogen(FB)and D-dimer and the whole blood high shear viscosity in the observation group were lower than before treatment and the control group(P<0.05);and the whole blood low shear viscosity,the capillary blood viscosity,packed cell volume and red cell assembling index were all lower than the control group(P<0.05).Conclusion Xiao Ai Ping injection can effectively improve the blood high coagulation state and anticancer efficacy of patients with advanced NSCLC,which would be recommended for the clinical used as an effective treatment.
