胸腔镜肺叶切除术与常规开胸手术治疗肺癌的疗效及并发症发生情况

摘　　要：目的对肺癌采用胸腔镜肺叶切除术治疗与常规开胸治疗的效果进行对比。方法选取94例肺癌患者,随机分为观察组与对照组,各47例,对照组采用开胸手术治疗,观察组采用胸腔镜肺叶切除术治疗,对比两组治疗效果。结果在手术时间、术中出血量、引流管引流量、引流管留置时间、术后疼痛时间及下床活动时间方面,观察组各指标均明显低于对照组,差异有统计学意义,P <0. 05;手术前后两组心肺功能指标水平对比,术后两组FVC、FEV1、PEF指标均较术前明显改善(P <0. 05),且观察组改善程度明显优于对照组,差异有统计学意义,P <0. 05;术后并发症发生率观察组为4. 26%,对照组为23. 40%,观察组明显低于对照组,差异有统计学意义,P <0. 05。结论对肺癌患者采用胸腔镜肺叶切除术治疗,具有创伤小、术后恢复快等优势,可有效改善患者心肺功能,且术后并发症较少,手术安全性高,在临床中有推广应用的价值。Objective To compare the effect of thoracoscopic lobectomy and conventional thoracotomy for lung cancer.Methods 94 patients with lung cancer were selected,and were randomly divided into the observation group and the control group,there were 47 cases in each group. The control group was treated with thoracotomy,and the observation group was treated with thoracoscopic lobectomy,then the therapeutic effects of the 2 groups were compared. Results Operation time,intraoperative blood loss,drainage volume,drainage tube indwelling time,postoperative pain time and time of getting out of bed of the observation group were significantly lower than those of the control group( P < 0. 05);The cardiopulmonary function indexes between the 2 groups before and after surgery were compared,the indexes of FVC,FEV1 and PEF in the 2 groups before surgery were significantly improved than those after surgery( P < 0. 05),and the degree of improvement in the observation group was significantly better than that in the control group( P < 0. 05);On the incidence of postoperative complications,observation group was 4. 26%,the control group was 23. 40%,that of the observation group was significantly lower than that of the control group( P < 0. 05).Conclusion Patients with lung cancer treating with thoracoscopic lobectomy has advantages of small trauma and quick postoperative recovery,which can effectively improve the cardiopulmonary function of patients,the postoperative complications caused by which are few and the operation is safe,it has the value of popularizing and applying in clinic.
