后腹腔镜肾癌根治术与开放性肾癌根治术对肾癌的近期疗效及心肌酶谱的影响

摘　　要：目的探讨后腹腔镜肾癌根治术与开放性肾癌根治术对肾癌患者的近期疗效及心肌酶谱的影响。方法回顾性选择85例肾癌患者,根据手术方法的不同,分为观察组(43例)及对照组(42例),对照组患者给予开放性肾癌根治术,观察组患者给予后腹腔镜肾癌根治术,对比两组患者的围术期指标,两组患者手术前后1 d的TNF-α、IL-6、CRP水平,两组患者术前、术后1 d、术后3 d的肌酸激酶、肌酸激酶同工酶、乳酸脱氢酶及术后并发症。结果观察组的手术时间明显长于对照组,但术中出血量、术后肠道恢复时间、住院时间、并发症发生率明显优于对照组,P<0.05。术前,两组患者的TNF-α、IL-6、CRP水平对比无统计学意义,P>0.05;术后,两组患者的以上指标均明显上升,且对照组明显高于观察组,P<0.05。术前、术后1 d、3 d,对照组的肌酸激酶、肌酸激酶同工酶、乳酸脱氢酶对比均无统计学意义,P>0.05;术后1 d,观察组以上指标均明显较术前高,P<0.05,术后3 d,观察组的以上指标与术前对比无统计学意义,P>0.05;术后1 d时,观察组以上指标明显高于对照组,P<0.05;术前及术后3 d,两组以上指标对比无统计学意义,P>0.05。结论后腹腔镜肾癌根治术对患者较开放性肾癌根治术对患者的创伤小,术后并发症及炎症反应少,但会对患者的心肌酶谱产生一定影响,但影响时间短,可快速恢复。Objective To investigate the effect of retroperitoneal laparoscopic radical nephrectomy and open radical nephrectomy on short-term efficacy and myocardial enzyme profile of patients with renal cancer.Methods 85 cases with renal cancer were retrospective selection,who were divided to the observation group(43 cases)and the control group(42 cases).The control group were given open radical nephrectomy,the observation group were given retroperitoneoscopic radical nephrectomy,the perioperative indicators of the 2 groups,the TNF-α、IL-6、CRP level before and after surgery for 1 d,the creatine kinase,creatine kinase isoenzyme,lactate dehydrogenase of the 2 groups before and after surgery for 1 d,3 d of the 2 groups were compared,the complications of the 2 groups after surgery were compared.Results The operation time of the observation group was longer than the control group,P<0.05,the intraoperative blood loss,postoperative intestinal normality,hospitalization time of the observation group were better than the control group,P<0.05.The TNF-α、IL-6、CRP level of the 2 groups before surgery had no significant difference,P>0.05.After surgery,the indexes were higher than before surgery,and the control group were higher than the observation group,P<0.05.The creatine kinase,creatine kinase isoenzyme,lactate dehydrogenase of the control group before surgery,after surgery for 1 d and 3 d had no significant difference,P>0.05,after surgery for 1 d,which in the observation group were higher than before surgery and the control group,after surgery for 3 d,which had no significant difference with after surgery for 1 d,before surgery and after surgery for 3 d,which had no significant difference in the 2 groups,P>0.05.Conclusion Compared with open radical nephrectomy,retroperitoneal laparoscopic radical nephrectomy have less trauma to patients,less postoperative complications and inflammatory reactions,which will have a certain impact on the myocardial enzyme profile of patients,while the impact time is short and could be recovered
