内镜下放置覆膜食管支架治疗晚期食管癌癌性梗阻成功植入率的分析

摘　　要：目的探究内镜下放置覆膜食管支架治疗晚期食管癌癌性梗阻成功植入率的影响。方法选取75例晚期食管癌癌性梗阻患者,进行内镜下放置覆膜食管支架治疗方法,作为观察组,选取同期放弃内镜治疗的30例患者,作为对照组,进行回顾性分析。观察患者的支架置入成功率和并发症发生情况,生存期以及生存质量情况。结果采用内镜下放置覆膜食管支架治疗的患者成功植入率为97. 33%,患者吞咽困难问题得到明显缓解。进行内镜下放置覆膜食管支架治疗的观察组,6个月、12个月和18个月的累积生存率分别为98. 67%、89. 33%、69. 33%,放弃内镜治疗的观察组分别为53. 3%、36. 67%、23. 33%,两组存在明显差异(P <0. 05),具有统计学意义。观察组的生存质量评分(QOL)明显高于对照组,两组存在明显差异(P <0. 05),具有统计学意义。结论内镜下放置覆膜食管支架治疗晚期食管癌癌性梗阻成功植入率高,能够明显改善患者的症状,提高近期生存率和生存质量。Objective To explore The effect of successful implantation rate of covered esophageal stent in the treatment of advanced esophageal carcinoma carcinomatous obstruction under endoscope. Methods 75 patients with malignant obstruction of advanced esophageal cancer were treated with endoscopic covered esophageal stent placement as the observation group. 30 patients who abandoned endoscopic therapy at the same time were selected as the control group for retrospective analysis. The success rate and complications of stent implantation,survival time and quality of life were observed. Results The successful implantation rate of covered esophageal stent in the treatment of patients with advanced esophageal carcinoma under endoscope was97. 33%,the dysphagia was obviously relieved. The observation group who was treated with coated esophageal stent under endoscope,the cumulative survival rates at 6,12 and 18 months were 98. 67%,89. 33% and 69. 33%,respectively;The observation group who gave up endoscopic treatment,the cumulative survival rates were 53. 3%,36. 67% and 23. 33%,respectively( P <0. 05). The quality of life score( QOL) of observation group was significantly higher than these of the control group( P < 0. 05).Conclusion Covered esophageal stent in the treatment of advanced esophageal carcinoma carcinomatous obstruction under endoscope has high successful implantation rate,which can significantly improve the patient’s symptoms,and improve the long term survival rate and quality of life.
