p16在宫颈上皮内瘤变诊断中的分流作用

摘　　要：目的分析p16蛋白在不同级别宫颈上皮内瘤变组织中的表达差异及其在分流管理中的价值。方法收集由活检、宫颈锥切或手术切除子宫的宫颈病变组织标本118例作为实验组(CINⅠ43例,CINⅡ27例,CINⅢ48例,CIN为宫颈上皮内瘤变),良性疾病切除子宫后的宫颈病变组织25例作为对照组。采用免疫组织化学法检测所有宫颈组织标本中p16蛋白的表达情况,结果进行统计学分析。结果p16蛋白阳性表达率在不同级别子宫颈鳞状上皮内病变组织中呈现递增趋势;实验组与对照组比较差异有统计学意义(P<0.05);实验组内两两比较差异亦有统计学意义(P<0.05)。结论p16蛋白的表达与宫颈上皮内瘤变的分级呈现相关性,可作为宫颈病变分层管理临床指标。Objective To analyze the expression of P16 protein in different levels of cervical intraepithelial neoplasia and its diffluent function in the auxiliary diagnosis.Methods 118 patients with CIN were selected in the study and all the patients with cervical biopsy specimens or conization were collected.According to the pathologic results,the patients were divided into CIN I group with 43 cases,CIN II group with 27 cases,CIN III group with 48 cases and 25 cases of benign lesions.The expressions of P16 protein in each group were measured by immunohistochemistry and the data was analyzed.Results The positive expression rate of P16 protein in different levels of cervical squamous intraepithelial lesions showed an increasing trend.Compared with the control group,the difference was statistically significant(P<0.05).The differences were also statistically significant in the experimental group(P<0.05).Conclusion The expression of P16 protein is correlated with the grading of cervical intraepithelial lesions,which could be used as a clinical indicator for stratified management cervical lesions.
