HFABP在急性缺血心肌中的表达及在法医病理诊断中的意义

摘　　要：目的探讨心型脂肪酸结合蛋白(HFABP)在急性缺血心肌中的表达及在法医病理诊断中的意义。方法按条件选取心肌组织标本30例,分为对照组、冠脉狭窄组、心肌梗死组各10例。肉眼观察冠状动脉左前降支及心肌组织的变化;HE染色方法观察各组心肌组织的形态变化,免疫组织化学方法观察各组心肌组织中HFABP的表达。结果肉眼及HE染色显示:冠脉狭窄组、心肌梗死组与对照组相比左前降支狭窄程度明显。HE结果显示:心肌梗死组心肌细胞排列紊乱,部分心肌有断裂,细胞核固缩、凝集、深染,并有核碎裂、核溶解;冠脉狭窄组心肌细胞核部分固缩、凝集、碎裂,与对照组心肌细胞无明显区分。免疫组化结果显示:对照组HFABP阳性表达平均光密度值0.26±0.01,冠脉狭窄组HFABP阳性表达平均光密度值0.19±0.005,心肌梗死组HFABP阳性表达平均光密度值0.14±0.01。结论心肌梗死组HFABP的表达明显弱于冠脉狭窄组,冠脉狭窄组明显弱于对照组。急性缺血心肌中HFABP的表达量明显减少。Objective To investigate the expression of HFABP in acute ischemic myocardium and its significance in forensic pathological diagnosis.Methods 30 cases of myocardial tissue specimens were selected according to the conditions,and there were 10 cases in control group,10 cases in coronary stenosis group and 10 cases in myocardial infarction group.The left anterior descending coronary artery and myocardial tissue changes were observed under macroscopic.Morphological changes of myocardial tissue in each group were observed by HE staining.The expression of HFABP in myocardial tissue of each group was observed by immunohistochemistry.Results The macroscopic and HE staining indicated that the degree of stenosis of left anterior descending artery in coronary stenosis group and myocardial infarction group was significantly higher than that in control group.And the HE staining showed that in myocardial infarction group,the arrangement of myocardial cells was disordered,some of the myocardium was broken,the nuclei were condensed,hyperchromatic,fragmented and dissolved;in the coronary artery stenosis group,the nuclei were condensed,agglutinated and fragmented,but there was no significant difference between the myocardial cells in the control group and that in the coronary artery stenosis group.Immunohistochemical results showed that average optical density of the positive expression of HFABP was 0.26±0.01 in control group,the average optical density of the positive expression of HFABP was 0.19±0.005 in the coronary artery stenosis group and the average optical density of the positive expression of HFABP was 0.14±0.01 in the myocardial infarction group.Conclusion The expression of HFABP in the myocardial infarction group is significantly weaker than that in the coronary stenosis group and the coronary stenosis group.The expression of HFABP in stenosis group is significantly weaker than that in control group.The expression of HFABP in acute ischemic myocardium is obviously decreased.
