非侵入性检查在乳房外Paget病的诊断及治疗中的应用进展

摘　　要：乳房外Paget病是一种少见的上皮性皮肤恶性肿瘤,临床表现为红斑、糜烂,易被误诊为湿疹、真菌感染,因而延误治疗。目前诊断的金标准是皮肤组织病理学,而肿瘤的分期依赖于前哨淋巴结活检,但由于这两项检查均为有创检查,在临床应用中因患者有时无法接受而使应用受到限制。此外,在手术过程中,快速病理为确定手术边缘的常用方法,但它具有时间长、手术切缘阳性可能等不足。如今随着非侵入性影像学检查在皮肤科越来越广泛的应用,其在乳房外Paget病的诊断与治疗中的应用也被积极推广,并且取得了一定的成果,本文就此进行阐述。Extramammary Paget's disease is an uncommon cutaneous malignancy.Clinically,tumors may appear non-specific,leading to delays in diagnosis and management.The diagnostic gold standard is biopsy with histological confirmation and the assessment of tumor stage depends on sentinel lymph node biopsy,which can not be widely used for unacceptable patients due to its invasiveness.In addition,rapid pathology is a common method for determining the margin of surgery during surgery.However,it needs more time and the surgical margin may be positive.Nowadays,with the increasing use of non-invasive imaging examinations in dermatology,its application in the diagnosis and management of extramammary Paget's disease has been actively promoted and achieved certain results.This paper summarized its significance in clinical practice.
