摘　　要：目的:研究乳腺区段切除联合腋窝淋巴结清扫术对早期乳腺癌患者生命质量及预后的影响。方法:选取某院收治的早期乳腺癌患者66例,采用随机数字表法分为对照组和观察组,每组各33例。对照组行乳腺切除术加腋淋巴结清扫术,观察组行乳腺区段切除术联合腋淋巴清扫术。观察并比较2组患者术后生命质量、并发症发生率、术后3年生存率、复发率及远处转移率和保留乳房的美观效果。结果:2组患者术后3个月生活质量评分均上升,且观察组心理因子、躯体因子、信仰因子及社会因子评分高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,术后生存率高于对照组,差异有统计学意义(P<0.05),2组患者远处转移率及复发率比较差异无统计学意义(P>0.05)。结论:早期乳腺癌患者行乳腺区段切除联合腋淋巴结清扫术疗效确切,不仅可以提高患者术后生命质量,且可以提高术后生存率,降低术后并发症发生率。Objective:To study the effect of segmental mastectomy combined with axillary lymph node dissection on quality of life and prognosis in patients with early breast cancer.Methods:66 patients with early breast cancer were selected and randomly divided into a control group and an observation group with 33 cases in each group.The ccontrol group underwent axillary lymph node dissection,and the observation group was treated with segmental mastectomy based on the regime of the control group.The postoperative quality of life,incidence of complications,postoperative 3-year survival rate,recurrence rate,distant metastasis rate and aesthetic effects of preserving breast were compared between the two groups.Results:3 months after surgery,the score of quality of life in both groups increased.The scores of mental factor,body factor,faith factor and social factor in the observation group were higher than those in the control group(P<0.05);the incidence of postoperative complications was lower and the postoperative survival rate was higher in the observation group than those in the control group(P<0.05).There were no statistical difference in the distant metastasis rate and rate of recurrence between the two groups(P>0.05).Conclusion:Segmental mastectomy combined with axillary lymph node dissection in treatment of patients with early breast cancer is effective.It can increase postoperative quality of life,improve the prognostic survival rate,and decrease incidence of postoperative complications.
