多发性骨髓瘤患者转录因子HOXB7表达水平及临床意义

摘　　要：目的探讨多发性骨髓瘤(MM)患者转录因子同源异型盒基因B7(HOXB7)表达水平及临床意义。方法选择2015年1月—2016年12月本院收治的MM患者94例作为MM组,另选同期在本院进行体检的健康体检者30名作为对照组,应用实时荧光定量PCR法检测HOXB7 mRNA的表达,Western blot法检测其蛋白表达水平,比较两组HOXB7表达的差异,分析HOXB7表达与MM患者临床分期、病情分期的关系,并应用受试者工作特征曲线(ROC)评价HOXB7表达对MM的诊断效能。结果MM组HOXB7 mRNA及蛋白表达水平均明显高于对照组(P<0.05);随着临床分期提高,HOXB7 mRNA及蛋白表达水平逐渐显著增高(P<0.05),且病情分期为进展期患者HOXB7 mRNA及蛋白表达水平显著高于稳定期患者(P<0.05)。Pearson相关分析显示,MM患者HOXB7 mRNA及蛋白表达均与临床分期、病情分期均成正相关(P<0.05)。高水平(>0.350)HOXB7 mRNA患者中位生存期为15.7个月,明显低于低水平(≤0.350)HOXB7 mRNA患者的26.8个月。结论HOXB7表达上调与MM患者疾病发生、进展及预后密切相关。Objective To explore the expression level and clinical significance of transcription factor homeobox gene B7(HOXB7)in patients with multiple myeloma(MM).Methods 94 patients with MM(MM group)admitted to our hospital during January 2015 and December 2016 and 30 healthy examiners(control group)were selected.The expression of HOXB7 mRNA was detected by real-time fluorescent quantitative PCR.The protein expression level was detected by Western blot.The difference of HOXB7 expression was compared between the two groups.The relationship between HOXB7 expression and clinical staging and disease staging of MM patients was analyzed,and the receiver operating characteristic curve(ROC)was used to evaluate the diagnostic efficacy of HOXB7 expression on MM.Results The mRNA and protein expression levels of HOXB7 in MM group was significantly higher than that in control group(P<0.05).With the increase of clinical staging,the mRNA and protein expression levels of HOXB7 were significantly increased(P<0.05),and the mRNA and protein expression levels of HOXB7 of patients in progressive stage were significantly higher than those of patients in stable stage(P<0.05).Pearson correlation analysis showed that the expressions of HOXB7 mRNA and protein in MM patients were positively correlated with clinical staging and diseases staging(P<0.05).The median survival time of patients with high level(>0.350)of HOXB7 mRNA was significantly lower than that of patients with low level(≤0.350)of HOXB7 mRNA(15.7 months vs 26.8 months).Conclusions Up-regulation of HOXB7 expression is closely related to the occurrence,progression and prognosis of MM patients.
