摘 要:目的:探究三维数字减影血管造影(3D-DSA)在颅内动脉瘤的诊断价值。方法:选择医院收治的88例疑似颅内动脉瘤患者,所有患者均行常规二维数字减影血管造影(2D-DSA)检查后再行3D-DSA检查,比较两种检查方式对不同大小、不同位置的动脉瘤检出率,评价瘤颈及瘤颈与载瘤动脉间的关系显示情况,比较对比剂用量。结果:经2D-DSA和3D-DSA检查,检出动脉瘤分别为76例(88枚)和82例(98枚),诊断准确率分别为89.80%和100%;2D-DSA与3D-DSA对动脉瘤诊断准确率比较,差异有统计学意义(x^2=8.536,P<0.05);3D-DSA对动脉瘤瘤颈和瘤体与载瘤动脉关系显示率均显著高于2D-DSA,其差异有统计学意义(x^2=59.566,x^2=72.554;P<0.05);2D-DSA检查患者平均对比剂用量显著高于3D-DSA检查,其差异有统计学意义(t=11.978,P<0.05)。结论:3D-DSA可提高动脉瘤检出率,清晰显示瘤颈及瘤颈与载瘤动脉的关系,并可减少检查中对比剂的使用量,提高患者在检查中的安全性。Objective: To investigate the diagnostic value of three-dimensional digital subtraction angiography(3D-DSA) for intracranial aneurysms. Methods: 88 patients with suspected intracranial aneurysms who admitted to the hospital were enrolled in the study. After all patients underwent routine 2D-DSA, they underwent 3D-DSA. The detection rates of aneurysms with different sizes and positions of the two examination methods were compared. The displays for neoplasia neck, and the relationship between neoplasia neck and parent artery were evaluated. And the dosage of contrast agent between the two methods was further compared. Results: There were 76 cases(88 arterial aneurysms) and 82 cases(98 arterial aneurysms) with aneurysms were diagnosed by 2D-DSA and 3D-DSA, respectively. The diagnostic accuracies of the two methods were 89.80% and 100%, respectively. The difference of diagnostic accuracy for artery aneurysms between 2D-DSA and 3D-DSA was significant(x^2=8.536, P<0.05). The display rates of 3D-DSA for neoplasia neck, and the relationship between neoplasia neck and parent artery were significantly higher than those of 2D-DSA(x^2=59.566, x^2=72.554, P<0.05). The dosage of average contrast agents of 2D-DSA was significantly higher than that of 3D-DSA(t=11.978, P<0.05). Conclusion: 3D-DSA can improve the detection rate of aneurysms, and clearly display neoplasia neck and the relationship between neoplasia neck and parent artery, and reduce the dosage of contrast agent in examination, and improve safety during the examination.
