摘  要:目的:分析脊柱肿瘤切除后脊柱重建使用3D打印技术的效果。方法:选取医院收治的77例脊柱肿瘤切除后脊柱重建患者资料,将使用3D打印技术的患者纳入观察组(32例),使用常规脊柱重建术患者纳入对照组(45例);比较两组患者手术基本情况、手术前后视觉模拟量表(VAS)评分及脊髓损伤Frankel分级、术后并发症发生率以及术后随访情况。结果:观察组患者手术时间、出血量和住院时间显著低于对照组患者,差异有统计学意义(t=12.315,t=56.978,t=2.860;P<0.05);两组手术成功率比较差异无统计学意义;两组患者手术后与手术前比较,VAS评分下降,Frankel分级升高,差异具有统计学意义(t观察组=20.957,t=-6.273,t=25.195、t=-14.995;P<0.05);手术后两组患者VAS评分以及Frankel分对照组级差异无统计学意义;观察组患者术后并发症发生率为15.63%,低于对照组并发症发生率(35.56%),差异有统计学意义(x2=4.04,P<0.05);两组患者随访半年,患者相关脊椎疼痛症状减轻,运动功能改善,肿瘤无复发,植入假体无松动且与患者自身脊椎融合良好。结论:3D打印技术用于脊柱肿瘤切除脊柱重建安全可行,手术效果优于常规脊柱重建,并发症发生减少。Objective:To analyze the feasibility of using 3D printing technique to reconstruct spine after spinal tumors was excised.Methods:The data of 77 patients who underwent spinal re-construction after spinal tumors were excised in the hospital were retrospectively analyzed.Patients were treated by 3D printing technique were included in observation group(32 cases)and those were treated by conventionally spinal reconstruction were included in control group(45 cases).The basic conditions of surgery,visual analogue scale(VAS)scores and Frankel grades of spinal cord injury before and after surgery,incidence of postoperative complications and postoperative follow-up results between the two groups were compared.Results:The operation time,amount of bleeding and hospital stay of observation group were significantly shorter/less than those of control group(t=12.315,t=56.978,t=2.860,P<0.05).There was no significant difference in the success rate of surgery between the two groups.Compared with pre operation,VAS scores of both groups post operation were significantly decreased,while Frankel grades of both groups post operation were significantly increased(tobservation group=20.957,t=-6.273,tcontrol group=25.195,t=-14.995,P<0.05).There were no significant differences in VAS scores and Frankel grade between the two groups after surgery.The incidence of postoperative complications in observation group(15.63%)was significantly lower than that in control group(35.56%)(x2=4.04,P<0.05).All patients were followed up for half a year,and the results showed that the spinal pain symptoms of them were relieved,and motor function of them was improved,and tumors of them weren’t recurrence and implanted prosthesis were not loosen,which was well integrated with the patient's own spine.Conclusion:3D printing is safe and feasible for spinal reconstruction after excision of spinal tumors.The surgical effect of 3D printing technique is better than that of conventional spinal reconstruction,and complications of 3D printing technique are fewer.
