摘　　要：目的探讨电视辅助胸腔镜手术(VATS)对食管平滑肌瘤患者的疗效及其对患者术后炎症指标的影响。方法收集接受手术治疗的120例食管平滑肌瘤患者临床资料,根据手术方式分为VATS组(研究组)和胸后外侧改良小切口切除术组(对照组)。对比2组患者的围术期指标、手术相关不良反应及术后炎症因子水平。结果与对照组相比,研究组的手术切口较小、出血量较少、手术时间及住院时间均较短,术后的血清IL-6、CRP及TNF-α水平均较低(P均<0.05)。术后2组患者的恶心呕吐、上腹部疼痛、呼吸困难发生率比较差异均无统计学意义(P均>0.05)。结论 VATS治疗食管平滑肌瘤对患者的创伤小,术后炎症反应较轻,有利患者快速康复。Objective To evaluate the clinical efficacy of video-assisted thoracoscopic surgery(VATS) in the treatment of esophageal leiomyoma and the effect upon postoperative inflammatory cytokines. Methods Clinical data of 120 patients with esophageal leiomyoma undergoing surgery were collected. All patients were divided into the video-assisted thoracoscopic surgery group(VATS, study group) and modified posterolateral thoracotomy group(control group). Perioperative parameters, surgical adverse events and the levels of postoperative inflammatory cytokines were statistically compared between two groups. Results In the study group, the surgical incision was significantly smaller, the blood loss was considerably less, the operation time and length of hospital stay were considerably shorter, and the postoperative levels of IL-6, CRP and TNF-α were evidently lower compared with those in the control group(all P<0.05). The incidence of nausea, vomiting, upper abdominal pain and dyspnea did not significantly differ between two groups(all P>0.05). Conclusion VATS causes mild trauma, slight inflammatory response and accelerates postoperative recovery of patients with esophageal leiomyoma.
