摘  要:目的探讨首发急性脑梗死(ACI)大动脉粥样硬化型(LAA)患者伴肾功能损伤的相关危险因素。方法收录2017年2月到2018年12月在河北医科大学第二医院神经内科住院接受诊治的首发ACI患者,TOAST分型筛选出LAA型患者123例,肾功能是否损伤分为肾功能损伤组(CysC>1.03mg·L^-1,病例组)和肾功能正常组(CysC<1.03mg·L^-1,对照组);收集患者的一般临床资料、化验室指标、相关辅助检查等,并对可能的相关危险因素进行单因素和多因素分析。结果首发ACI-LAA型患者123例,合并肾功能损伤41例,占比为33.33%,经单因素分析结果显示年龄、糖尿病差异有统计学意义(P<0.05);经相关混杂因素校正后,Logistic回归分析年龄、糖尿病差异有统计学意义(P<0.05),为首发ACI-LAA型伴发肾功能损伤患者的独立危险因素。结论年龄、糖尿病与首发ACI-LAA型患者肾功能损伤的发生相关。Objective A goal was to explore the risk factors associated with renal function impairment in patients with LAA who were first acute cerebral infarction(ACI). Methods The first acute cerebral infarction patients admitted to the Department of Neurology, Second Hospital of Hebei Medical University from February 2017 to December 2018 were enrolled. According to the classification of TOAST etiology, 123 patients with LAA type were screened.According to whether the renal function was damaged,the patients were divided into renal function injury group(CysC>1.03 mg·L^-1, case group) and normal renal function group(CysC<1.03 mg·L^-1, control group);General clinical data, laboratory indicators, related auxiliary examination and other information were collected. Univariate and multivariate analyses were used to analyze possible risk factors. Results According to the inclusion criteria,a total of 123 patients with LAA-type acute cerebral infarction were included in the study, and 41 patients with renal dysfunction, accounting for33.33%. The results of single factor analysis showed that the difference in age and diabetes was statistically significant(P<0.05). After correction of relevant confounding factors, logistic regression analysis showed that the age and diabetes differences were statistically significant(P<0.05), which was an independent risk factor for patients with LAA-type renal impairment with acute cerebral infarction. Conclusion Age、diabetes is associated with the occurrence of renal impairment in LAA patients with first-episode acute cerebral infarction.
