依达拉奉联合丙戊酸钠治疗脑卒中后癫痫的效果

摘　　要：目的探讨依达拉奉联合丙戊酸钠治疗脑卒中后癫痫(PSE)的效果。方法回顾性分析2016年1月~2018年3月天津市环湖医院收治的102例PSE患者的临床资料,根据治疗方法的不同将患者分为对照组(55例)和研究组(47例)。对照组给予丙戊酸钠治疗,研究组在对照组的基础上联合依达拉奉治疗。比较两组患者临床疗效、癫痫发作次数、发作持续时间、神经元特异性烯醇化酶(NSE)水平、炎性因子及不良反应。结果研究组治疗后的临床总有效率高于对照组(P<0.05)。与治疗前比较,两组治疗后癫痫发作次数减少,发作持续时间缩短,且研究组治疗后癫痫发作次数少于对照组,发作持续时间短于对照组(P<0.05)。与治疗前比较,两组治疗后血清白介素-1β(IL-1β)、白介素-2(IL-2)、肿瘤坏死因子-α(TNF-α)水平均降低,且研究组治疗后低于对照组(P<0.05)。两组治疗3、6、12个月后血清NSE水平组间及组内比较差异无统计学意义(P>0.05);研究组治疗1个月后血清NSE水平低于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论依达拉奉联合丙戊酸钠治疗PSE可迅速改善临床症状,降低NSE水平,疗效确切,同时还可降低炎性因子水平,不增加不良反应发生率。Objective To investigate the efficacy of Edaravone combined with Sodium Valproate in the treatment of post-stroke epilepsy(PSE).Methods The clinical data of 102 patients with PSE who were treated in Tianjin Huanhu Hospital from January 2016 to March 2018 was analyzed retrospectively.According to the different treatment methods,the patients were divided into control group(55 cases)and study group(47 cases).The control group was treated with Sodium Valproate,and the study group was treated with Edaravone on the basis of the control group.The clinical efficacy,seizure frequency,duration,levels of neuron specific enolase(NSE),inflammatory factors and adverse reactions were compared between the two groups.Results The total effective rate of the study group was significantly higher than the control group(P<0.05).Compared with those before treatment,the number of epileptic seizures decreased and the duration of epileptic seizures shortened in the two groups after treatment.After treatment,the number of epileptic seizures in the study group was less than the control group,and the duration of epileptic seizures was shorter than the control group(P<0.05).Compared with those before treatment,the levels of serum interleukin-1β(IL-1β),interleukin-2(IL-2)and tumor necrosis factor-α(TNF-α)in the two groups were decreased,after treatment,which in the study group were lower than the control group after treatment(P<0.05).There was no significant difference in serum NSE level between the two groups after 3,6 months and 12 months of treatment(P>0.05).The serum NSE level in the study group was lower than the control group after 1 month of treatment(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Edaravone combined with Sodium Valproate in the treatment of PSE can rapidly improve clinical symptoms and reduce the level of NSE with definite curative effect.It can also reduce the level of inflammatory factors without increasing the incidence of adverse reacti
