摘  要:目的探讨阿魏酸钠对颈动脉易损斑块患者Toll样受体4(TLR4)/核因子-κB(NF-κB)信号通路及血清基质金属蛋白酶7(MMP-7)、基质金属蛋白酶9(MMP-9)的影响。方法收集轻型缺血性脑卒中且颈动脉超声检查提示存在易损斑块的患者174例,采用随机数字表法分为实验组和对照组,每组87例。对照组予抗血小板聚集,降脂,控制血压、血糖等常规治疗,实验组在常规治疗的基础上同时口服阿魏酸钠片100mg/次,3次/d。两组均治疗8周。采用ELISA法检测所有患者血清超敏-C反应蛋白(hs-CRP)、白细胞介素6(IL-6)、TLR4、NF-κB、MMP-7和MMP-9水平。结果治疗后实验组患者血清hs-CRP、IL-6、TLR4、NF-κB、MMP-7和MMP-9水平均较对照组下降,且两组患者治疗后血清hs-CRP、IL-6、TLR4、NF-κB、MMP-7和MMP-9水平均较治疗前下降,差异均有统计学意义(均P<0.05)。结论阿魏酸钠可能通过抑制颈动脉易损斑块TLR4/NF-κB的信号通路,降低炎症因子hs-CRP、IL-6水平和血清MMP-7、MMP-9的表达,从而可能增加颈动脉易损斑块的稳定性。Objective To investigate the effect of sodium ferulate on TLR4/NF-κB inflammatory signal pathway and the expression of serum MMP-7 and MMP-9 in patients with carotid vulnerable plaque.Methods One hundred and seventy-four patients with mild stroke and carotid vulnerable plaques were randomly divided into two groups with 87 cases in each group.Patients in control group were given routine treatment including anti-platelet aggregation,lipid-lowering,control of blood pressure and blood sugar,while those in study group were given sodium ferulate tablets(100 mg,t.i.d)on the basis of routine treatment.Both groups were treated for 8 weeks.The serum levels of of hs-CRP,IL-6,TLR4,NF-κB,MMP-7 and MMP-9 of the two groups were detected by enzyme-linked immunosorbent assay.Results After 8 weeks of treatment,the levels of hs-CRP,IL-6,TLR4,NF-κB,MMP-7 and MMP-9 in the two groups were lower than those before treatment(all P<0.05);while compared with the control group,the above indicators in study group were significantly decreased(all P<0.05).Conclusion Sodium ferulate may significantly inhibit the activation of TLR4/NF-κB signaling pathway and the inflammatory factors,reduce the expression of hs-CRP、IL-6、MMP-7 and MMP-9,which may increase the stability of carotid vulnerable plaque.
