摘  要:目的探讨脑小血管病(cerebral small vascular disease,CSVD)患者血清3-硝基酪氨酸(3-nitrotyrosine,3-NT)的含量及其与疾病认知障碍发生的相关性。方法选择CSVD患者55例,根据MoCA量表评分将患者分为伴认知功能障碍29例(cognitive impairmen-cerebral small vascular disease,CI-CSVD组),无认知功能障碍组26例(none cognitive impairment-cerebral small vascular disease,NCI-CSVD组),选用同期健康体检者51例作为健康对照组(health controls,HC组)。ELISA法检测外周血清3-NT含量。t检验分析CSVD组与HC组血清3-NT差异,并进一步分析CI-CSVD组和NCI-CSVD组血清3-NT组间差异。应用logistic回归曲线分析CSVD的相关危险因素,并采用偏相关分析CSVD组血清3-NT与MoCA总分及各项评分的相关性。结果①CSVD组中血清3-NT明显高于HC组(P=0.001,P<0.01)。②CI-CSVD组中血清3-NT明显高于NCI-CSVD组(P=0.021,P<0.05)。③经多因素logistic回归分析结果显示,血清低密度脂蛋白(low density lipoprotein,LDL)、尿酸(uric acid,UA)和3-NT均为CSVD的危险因素。④CSVD组中血清3-NT与MoCA总分(r=-0.327,P=0.013)、视空间与执行功能(r=-0.312,P=0.011)、注意与计算力(r=-0.332,P=0.011)、抽象力(r=-0.267,P=0.023)、延迟回忆亚项(r=-0.301,P<0.05)评分呈负相关性。结论CSVD患者中具有较高的血清3-NT,伴认知障碍的CSVD患者中血清3-NT更高,血清3-NT与CSVD及其认知障碍密切相关。LDL、UA以及3-NT可能为CSVD发生的相关危险因素。Objective To investigate the relationship between 3-nitrotyrosine(3-NT)levels in serum and morbidity of cognitive impairment in cerebral small vessel disease patients(CSVD).Method Fifty-five patients of CSVD were divided into with cognitive impairment group(CI-CSVD)(29 cases)and none cognitive impairment group(NCI-CSVD)(26 cases)according to MoCA.Fifty-one health subjects served as health controls(HC).ELISA method was used to detect 3-NT levels in serum.T-test was used to analyze the differences in serum 3-NT between the CSVD group and the HC control group as well as between the CI-CSVD group and the NCI-CSVD group.Logistic regression analysis were performed to predict the risk factors of CSVD.Partial correlation analysis was performed to explore the relationship between 3-NT in serum and MoCA scores including each item scores in MoCA.Results①The 3-NT in CSVD group were significantly higher than that in control group(P=0.001,P<0.01).②The 3-NT in the CI-CSVD group were significantly higher than that in the NCI-CSVD group(P=0.021,P<0.05).③Logistic regression analysis showed that LDL,UA and 3-NT were risk factors of CSVD.④The 3-NT in the CSVD groups was negatively correlated with MoCA scores(r=-0.327,P=0.013),space visibility and executive functionality(r=-0.312,P=0.011),attention and calculating performance(r=-0.332,P=0.011),abstraction strength(r=-0.267,P=0.023),subitem of the delayed memory(r=-0.301,P<0.05).Conclusion CSVD,particularly those with cognitive impairment,has high level of serum 3-NT.Serum 3-NT is closely related to CSVD and CSVD-related cognitive impairment.Serum LDL and UA may be related risk factors for CSVD.
