摘　　要：目的探讨丝氨酸蛋白酶抑制剂Kazal1型(SPINK1)蛋白和组蛋白甲基化转移酶G9a在胃癌中的表达情况及其对患者预后的评估价值。方法选取我院2012年1月—2015年6月收治的112例胃癌患者,利用免疫组化法对手术切除的胃癌组织及其癌旁正常组织的SPINK1和G9a蛋白表达水平进行检测,并分析SPINK1和G9a蛋白表达与患者临床病理参数和预后的关系。结果SPINK1、G9a蛋白在胃癌组织中的阳性表达率高于癌旁正常组织,其表达水平与胃癌患者的肿瘤浸润、淋巴结转移、TNM分期相关(P<0.05,P<0.01)。胃癌组织中SPINK1和G9a表达呈正相关(r=0.831,P<0.05)。胃癌组织中SPINK1和G9a高表达患者3年存活率低于低表达者(P<0.01);SPINK1和G9a共高表达者中位生存时间短于共低表达者,无病生存率、总生存率低于共低表达者(P<0.05,P<0.01)。结论胃癌组织中SPINK1和G9a的高水平表达可能会促进患者的病程发展,是预测患者不良预后的危险因素。Objective To investigate expression of serine protease inhibitor kazal1 type 1(SPINK1)and histone methyltransferase G9a in gastric cancer and their evaluation value for prognosis of patients.Methods A total of 112 patients with gastric cancer admitted during January 2012 and June 2015 were enrolled in this study.Immunohistochemical staining was performed to detect expressions of SPINK1 and G9a proteins in resected gastric cancer tissues and adjacent normal tissues.Relationships between expressions of SPINK1 and G9a protein with clinical pathological parameters and prognosis were analyzed.Results Positive expression rates of SPINK1 and G9a proteins in gastric cancer were significantly higher than those in normal tissues,and the expressions of SPINK1 and G9a proteins were related to tumor infiltration,lymph node metastasis and TNM stage(P<0.05,P<0.01).There was a positive correlation between expressions of SPINK1 and G9a proteins in gastric cancer tissues(r=0.831,P<0.05).In gastric cancer tissues,3-year survival rate in patients with high expressions of SPINK1 and G9a proteins were significantly lower than those with low expressions of SPINK1 and G9a proteins(P<0.01);in patients with high expressions of SPINK1 and G9a proteins,median survival time was shorter,while disease-free survival rate and total survival rate were lower than those inpatients with low expressions of SPINK1 and G9a proteins(P<0.05,P<0.01).Conclusion High-level expressions of SPINK1 and G9a proteins in gastric cancer tissues may promote progress of disease,which are risk factors for predicting poor prognosis of patients.
