摘  要:目的探讨膳食模式与新生儿低出生体重(LBW)发生风险的关系。方法利用2013年"陕西省出生缺陷现状及其危险因素调查"的横断面调查资料,选取陕西省20个县和10个区年龄为0~1岁的婴幼儿及其母亲作为调查对象。采用因子分析和结构方程模型相结合的方法研究膳食模式与新生儿低出生体重的关系。结果新生儿LBW发生率3.1%,小于胎龄儿(SGA)发生率12.6%。因子分析得出3种膳食模式,结构方程模型显示,均衡模式可降低新生儿LBW和SGA的发生风险(其对体重受限的作用为-0.24),零食模式的膳食是新生儿发生LBW和SGA的危险因素(其对体重受限的作用为0.18),而素食模式与新生儿出生体重的关系不显著。logistic回归模型显示,均衡模式可以降低新生儿LBW(OR=0.75,95%CI:0.59~0.95)和SGA(OR=0.84,95%CI:0.77~0.94)的发生风险,零食模式T3水平可增加LBW(OR=1.61,95%CI:1.06~2.45)的发生风险。结论均衡模式和零食模式与新生儿出生体重有密切关系,膳食种类多样化、营养均衡化、减少加工食物的摄入,对降低新生儿低出生体重的发生风险具有重要意义。Objective To explore the association between dietary pattern and risk of low birth weight(LBW).Methods The information about women of childbearing age and their children was obtained through a survey on the status of birth defects and their risk factors in Shaanxi province.Infants aged 0 to 1 year and their mothers were selected as the object of this investigation.The relationship between dietary pattern and low birth weight was studied by using factor analysis and structural equation model.Results The incidence of neonatal LBW was 3.1%and small for gestational age(SGA)was 12.6%.Three kinds of dietary patterns are found in the factor analysis,and structural equation model shows that the equilibrium mode can reduce the risk of neonatal LBW and SGA(of its effect on limited weight of-0.24),snacks mode is a risk factor for neonatal LBW and SGA(its effect on limited weight of 0.18),and vegetarian mode and the relationship between neonatal birth weight is not significant.Logistic regression model shows that the equilibrium mode can reduce the neonatal LBW(OR=0.75,95%C/:0.59~0.95)and SGA(OR=0.84,95%CI:0.77〜0.94),the T3 level of snacks mode can increase the risk of LBW(OR=1.61,95%C/:1.06~2.45).Conclusion Equilibrium mode and snacks mode are closely related to neonatal birth weight.Dietary diversity,nutrition equalization,reducing the intake of processed foods is of great significance to reduce the risk of low birth weight newborn.
