慢性牙周炎肥胖患者饮食疗法的效果评估

摘　　要：目的评估饮食疗法带来的减肥效果是否能提高肥胖患者对牙周炎治疗疗效。方法分析该院2016年9月至2018年12月192例肥胖患者,并按照有无联合饮食疗法分为两组。所有患者在接受牙周炎治疗后,在基线和10周时评估牙周炎相关指标、人体测量指标和生化指标的差异和相关性。结果在进行标准的牙周炎治疗10周后,两组患者的牙周参数均有所改善,但饮食疗法组的平均牙周袋深度(PD,P=0.032)和PD为4~<6 mm部位的百分比降低水平明显高于非饮食疗法组。治疗后,饮食疗法组中补体因子3(C3,P<0.01)和肿瘤坏死因子-α(TNF-α,P=0.042)降低水平明显高于非饮食疗法组,且平均PD的变化百分比与C3的变化相关(r=0.454,P=0.044),PD 4~<6 mm的部位的变化百分比与TNF-α的变化相关(r=0.414,P=0.012)。结论饮食疗法所致的减肥可引起全身炎症水平的降低,从而提高牙周炎治疗的效果。Objective To assess whether diet therapy can improve the efficacy of treatment in obese patients with periodontitis.Methods A total of 192 obese patients were retrospectively enrolled in hospital and divided into two groups according to the presence of diet therapy.All patients were assessed for differences and correlations of periodontal parameters,anthropometric and biochemical indicators between two groups at baseline and 10 weeks after receiving periodontitis.Results After 10 weeks of standard periodontitis treatment,the periodontal parameters improved in both groups,but the mean probing depth(PD,P=0.032)and the percentage of sites with PD 4-<6 mm in the diet group was significantly higher than the non-diet group.After treatment,the reduction of complement factor 3(C3,P<0.01)and tumor necrosis factor-α(TNF-α,P=0.042)in the diet group was significantly higher than the non-diet group.The mean reduction of PD was associated with changes of C3(r=0.454,P=0.044)and PD 4-<6 mm was associated with TNF-α(r=0.414,P=0.012).Conclusion Weight loss caused by diet therapy can reduce the level of systemic inflammation,thus improving the effect of periodontal treatment.
