快速康复外科干预对绝经前期腹腔镜子宫肌瘤切除术患者康复效果和营养状况的影响分析

摘　　要：目的分析快速康复外科(fast track surgery,FTS)干预对绝经前期腹腔镜子宫肌瘤切除术患者康复效果和营养状况的影响。方法选取海南省人民医院2016年4月至2019年4月收治的124例绝经前期子宫肌瘤患者为研究对象,采用随机数表法将其分为观察组和对照组,每组各62例。所有患者均行腹腔镜子宫肌瘤切除术,对照组患者围术期给予常规干预,观察组患者给予基于FTS理念的围术期干预。比较两组患者围术期相关指标、并发症发生率、术前和术后7 d营养指标变化。结果观察组患者术后排气时间、术后排便时间、住院天数均显著短于对照组(均P<0.05),住院费用显著少于对照组(P<0.05),术后并发症发生率显著低于对照组(P<0.05),术后7 d白蛋白(albumin,ALB)和前白蛋白(prealbumin,PA)水平均显著高于对照组(均P<0.05)。观察组患者手术前后ALB、PA、血红蛋白(hemoglobin,Hb)水平比较差异均无统计学意义(均P>0.05)。对照组患者术后7 d ALB、PA、Hb水平均显著低于本组术前(均P<0.05)。结论FTS干预能够促进绝经前期腹腔镜子宫肌瘤切除术患者术后康复,减轻手术对其营养状况的不良影响,且安全性较好。Objective To analyze the effect of fast track surgery(FTS)intervention on the recovery and trophic status of premenopause patients undergoing laparoscopic myomectomy.Method From April 2016 to April 2019,124 premenopausal patients with hysteromyoma in Hainan General Hospital were selected as the research objects.They were divided into observation group and control group by random number table method,62 patients in each group,all patients were treated with laparoscopic myomectomy.Patients in control group were treated with routine intervention during the perioperative period.Patients in observation group were treated with perioperative intervention based on the concept of FTS.The related indexes of perioperative period,complication rate,preoperative and postoperative nutrition indexes were compared between the two groups.Result The time of exhaust,defecation and hospitalization in observation group were significantly shorter than those in control group(all P<0.05),the hospitalization cost was significantly lower than that in control group(P<0.05),and the incidence of postoperative complications was significantly lower than that in control group(P<0.05),the levels of albumin(ALB)and prealbumin(PA)7 days after operation were significantly higher than those in control group(all P<0.05).There were no significant differences in the levels of ALB,PA,hemoglobin(HB)before and after operation in observation group(all P>0.05).The levels of ALB,PA and Hb in control group 7 days after operation were signific antly lower than those before operation(all P<0.05).Conclusion FTS can promote the recovery of premenopausal patients after laparoscopic myomectomy,and reduce the adverse effects of surgery on their trophic status,the safety is better.
