醒脑缩泉汤治疗急性脑卒中合并脑性耗盐综合征所致低钠血症的临床观察

摘　　要：目的观察醒脑缩泉汤治疗急性脑卒中合并脑性耗盐综合征所致低钠血症的临床疗效。方法患者60例随机分为两组各30例,对照组采用西医常规治疗,治疗组在对照组基础上采用醒脑缩泉汤治疗,均治疗5 d。观察两组临床疗效,治疗前后血钠(Na^+)、血浆脑钠肽(BNP)、C反应蛋白(CRP)、D-二聚体(D-Di)水平,及美国国立卫生研究院卒中量表(NIHSS)评分、日常生活能力评定量表(Barthel指数)。结果两组治疗后血Na^+水平、Barthel指数均较治疗前显著升高(P<0.05),且治疗组高于对照组(P<0.05)。两组治疗后BNP、CRP、D-Di、NIHSS评分均较治疗前均降低(P<0.05),且治疗组低于对照组(P<0.05)。治疗组急性脑卒中及低钠血症总有效率均高于对照组(P<0.05)。结论醒脑缩泉汤治疗急性脑卒中合并脑性耗盐综合征所致低钠血症,能够恢复血Na+水平,恢复神经受损功能,提高患者生活质量及临床疗效。Objective:To observe the clinical efficacy of Xingnao Suoquan Decoction on hyponatremia caused by acute stroke combined with cerebral salt-consuming syndrome.Methods:60 patients were randomly divided into two groups,30 cases in each.The control group was treated with western medicine.The treatment group was treated with Xingnao Suoquan Decoction on that basis.Both groups were treated for 5 days.The levels of blood sodium(Na^+),BNP,CRP,D-DI,NIHSS and Barthel index were observed before and after the treatment.Results:After treatment,the blood Na^+ level and Barthel index were higher than those before treatment in the same group(P<0.05),and the treatment group was higher than the control group(P<0.05).The levels of plasma BNP,CRP,D-Di and NIHSS were lower than those in the same group before treatment(P<0.05),and the treatment group was lower than the control group(P<0.05).The total effective rate of acute stroke and hyponatremia in the treatment group was higher than that in the control group(P<0.05).Conclusion:Xingnao Suoquan Decoction on acute stroke with hyponatremia caused by cerebral salt-consuming syndrome can promote the recovery of blood sodium level and nerve damaged function,improve the quality of life and clinical efficacy.
