匀浆膳联合蛋白肽对脑卒中患者营养干预评价

摘　　要：目的制备适用于脑卒中合并2型糖尿病患者的匀浆膳,并观察自制匀浆膳联合蛋白肽在该类患者中的临床应用效果。方法依据脑卒中合并2型糖尿病患者特点制备匀浆膳并将制备好的匀浆膳应用于50例该类患者中,随机分为2组,匀浆膳加蛋白肽组每日给予1 890 kcal(1 kcal=4.186 kJ)自制匀浆膳并联合应用20 g蛋白肽,匀浆膳组每日给予1 890 kcal自制匀浆膳,观察4周,记录患者营养支持前和营养支持两周后、四周后的相关生化指标。结果每组患者的空腹血糖、餐后2 h血糖、总蛋白、白蛋白、前白蛋白、血红蛋白、肌酐、钠离子等在营养支持前、支持两周和四周后,差异具有统计学意义。对于前白蛋白指标,匀浆膳加蛋白肽组和匀浆膳组的差异具有统计学意义。结论自制匀浆膳联合蛋白肽有利于患者血糖控制、营养状况改善以及钠离子调节。Objective This paper aims to prepare the appropriate homogenized meal for stroke patients with type 2 diabetes, and to observe the clinical effect of homogenized meal combined with protein peptide in stroke patients with type 2 diabetes. Methods By means of referring to the disease characteristics, the homogenized meal can be determined. The homogenized meal diet was applied in 50 stroke patients with type 2 diabetes. 50 patients were randomly divided into two groups. Patients of homogenate diet with protein peptide group and of homogenate diet group were given 1 890 kcal(1 kcal=4.186 kJ)homogenate meals combined with 20 g protein peptide and 1 890 kcal homogenate meals everyday, respectively. The study lasted for 4 weeks. The condition of blood glucose and related biochemical indexes before and after nutritional support for 2 weeks and 4 weeks were recorded for each patient. Results Fasting blood glucose, 2 h postprandial blood glucose, total protein, albumin, prealbumin, hemoglobin, creatinine, and sodium ions in each group showed statistically significant differences before, 2 weeks after and 4 weeks after nutritional support. The pre-albumin index of homogenate diet with protein peptide group was better than that of homogenate diet group, and the difference was statistically significant. Conclusion The homogenized meal combined with the appropriate amount of protein peptides has the functions of blood glucose control, nutritional status improvement and sodium ion regulation for stroke patients with type 2 diabetes.
