应用微研磨膏联合美白剂与渗透树脂治疗着色型氟斑牙的效果观察

摘　　要：目的:探讨微研磨膏联合美白凝胶与渗透树脂治疗氟斑牙的疗效。方法:选取2018年1月~2019年6月在我院口腔科就诊的53例氟斑牙患者,随机分为观察组27例和对照组26例,观察组采用"微研磨膏+10%低浓度皓齿家庭美白凝胶+渗透树脂"治疗,对照组采用美白凝胶治疗,应用VITA比色板对治疗前后氟斑牙色阶进行治疗效果比对,采用视觉模拟量表(visual analogue scale,VAS)观察两组治疗过程中的疼痛状况和敏感程度。结果:观察组轻度、中度和重度氟斑牙患者的治疗显效率均明显高于对照组同样严重程度的患者(χ~2=57.700、63.709、89.710,P=0.000);观察组治疗后的牙面色度学阶显著低于对照组(t=-5.439,P<0.001)。在治疗后1周和6个月,观察组中度和重度患者的脱色率均明显大于对照组中度(χ~2=18.687、7.697,P=0.000、0.006)和重度患者(χ~2=10.473、5.192,P=0.001、0.023)。两组患者在治疗过程中的牙齿酸痛敏感程度和VAS评分无显著差异(P>0.05)。结论:"微研磨膏+10%低浓度皓齿家庭美白凝胶+渗透树脂"治疗不同严重程度氟斑牙美白效果更好,安全性高,值得临床上推广应用。Objective:To investigate the efficacy of micro abrasive paste combined with whitening gel and permeable resin in the treatment of dental fluorosis.Methods:A total of 53 patients with dental fluorosis who were treated in Department of Stomatology of our hospital from January 2018 to June 2019 were randomly divided into the observation group(27 cases)and the control group(26 cases).The observation group was treated with "micro grinding paste+10% low concentration white teeth family whitening gel+osmotic resin".The control group was treated with whitening gel,and the VITA color plate was used to compare the treatment effect of dental fluorosis before and after treatment.Visual analogue scale(VAS)was used to observe the pain and sensitivity of the two group.Results:The effective rate of treatment in patients with mild,moderate,and severe dental fluorosis in the observation group was significantly higher than that in the control group(χ~2=57.700,63.709,89.710,P=0.000),the chromaticity level of the teeth in the observation group was significantly lower than that in the control group(t=-5.439,P<0.001).At 1 week and 6 months after the treatment,the decolorization rate of moderate and severe patients in the observation group was significantly higher than moderate patients(χ~2=18.687,7.697, P=0.000,0.006)and severe patients(χ~2=10.473,5.192,P=0.001,0.023)in the control group.And there was no significant difference between the two groups in the sensitivity of toothache and VAS score(P>0.05).Conclusion:The technique of micro grinding paste+10% low concentration white teeth family whitening gel+penetration resin has better whitening effect and high safety for dental fluorosis,worthy of clinical application.
