玻璃纤维桩联合自体断冠再接术治疗儿童恒前牙冠折的临床效果观察

摘　　要：目的:探讨玻璃纤维桩联合自体断冠再接术治疗儿童恒前牙冠折的临床效果。方法:选取100例(111颗牙)恒前牙冠折儿童为研究对象;其中51例(56颗牙)采用复合树脂直接修复(对照组),49例(55颗牙)采用玻璃纤维桩联合自体断冠再接术(观察组)。术后6、12、18个月进行随访,比较两组患者成功率。根据折断部位进一步分为颈1/3折断组和中1/3折断组,并比较各组成功率。术后18个月时对患者进行满意度调查。结果:术后6、12、18个月观察组成功率均显著高于对照组(P<0.05);术后6、12、18个月,中1/3折断组中观察组和对照组成功率比较均无明显差异(P>0.05),而颈1/3折断组中,观察组成功率均显著高于对照组(P<0.05)。术后18个月观察组在牙体形态、颜色以及咬合功能等方面满意度均显著高于对照组(P<0.05)。结论:玻璃纤维桩联合自体断冠再接术治疗儿童恒前牙冠折成功率高,且患者满意度高。Objective:To investigate the clinical effect of glass fiber post combined with autologous debridement in the treatment of children with permanent anterior crown fracture.Methods:One hundred children(111 teeth)with permanent anterior teeth were selected as the study subjects.Among them,51 cases(56 teeth)in the control group were directly repaired with composite resin,and 49 cases(55 teeth)in the observation group were treated with fiberglass post combined with autologous broken crown.Follow-up was performed at 6,12 and 18 months after surgery,and the success rate of the two groups was compared.According to the fracture site,the components were divided into the middle 1/3 fracture group and the neck 1/3 fracture group,and the success rate of each group was compared.Satisfaction survey was performed on patients at 18 months after surgery.Results:At 6, 12, and 18 months after operation, the success rate of the observation group was significantly higher than that of the control group(P<0.05). In the middle 1/3 fracture group, there was no significant difference in the success rate between the observation group and the control group(P<0.05). However, in the neck 1/3 fracture group, the success rate of the observation group was significantly higher than that of the control group(P<0.05).At 18 months after operation,the satisfaction of the observation group in terms of tooth morphology,color and occlusion function was significantly higher than that of the control group respectively(P<0.05).Conclusion:Glass fiber post combined with auto-disconnected coronary artery replacement for the treatment of children with permanent anterior crown fracture has high success rate and patient satisfaction.
