摘　　要：目的应用声触诊组织量化(VTQ)技术测量肝硬化食管胃静脉曲张(EGV)患者接受"内镜下食管胃静脉套扎及硬化治疗"前后肝脏、脾脏剪切波速度(LSWV、SSWV),与内窥镜检查所示EGV程度对照,观察LSWV、SSWV评价临床干预肝硬化并发EGV的效果。方法对42例肝硬化并发中-重食管胃静脉曲张患者,于治疗前及4周后以VTQ测量其SSWV、LSWV值,记录并比较治疗前后内镜所示EGV程度及SSWV、LSWV变化。结果中、重度EGV患者SSWV值不同。治疗后4周SSWV值下降,以下降0.13 m/s为临界值判断手术有效性的敏感度和特异度分别为61.50%和100%;中、重度EGV患者间LSWV值及治疗前后差异均无统计学意义(P均>0.05);Sarin分型各型EGV之间SSWV、LSWV值差异无统计学意义(P均>0.05)。结论 VTQ测量SSWV评估临床干预中-重度EGV效果具有较高应用价值。Objective To observe the value of liver shear wave velocities(LSWV) and spleen shear wave velocities(SSWV) measured with virtual touch tissue quantification(VTQ) before and after endoscopic esophagogastric vein ligation and sclerotherapy in liver cirrhosis patients with esophagogastric varices(EGV) for evaluating clinical intervention effect compared with endoscopic grading of EGV. Methods SSWV and LSWV values of 42 liver cirrhosis complicated with EGV patients were measured using VTQ before and 4 weeks after endoscopic esophagogastric varices ligation and sclerotherapy. The changes of SSWV and LSWV, as well as the endoscopic grading of EGV were compared before and after treatment. Results SSWV values of moderate and severe EGV were different. SSWV values decreased 4 weeks after treatment. The sensitivity and specificity was 61.50% and 100%, respectively, when the critical value was 0.13 m/s. There was no significant difference of LSWV value between patients with moderate and severe EGV, nor before and after treatment(both P>0.05). Under Sarin’s classification of EGV, there was no significant difference of SSWV nor LSWV between different types(all P>0.05). Conclusion SSWV measured with VTQ technique has good application value in evaluating the effect of clinical intervention of liver cirrhosis patients with moderate to severe esophagogastric varices.
