摘  要:目的探讨血清脂联素(APN)水平与伴有糖代谢紊乱(GMD)的轻度血管性认知障碍(mVCI)的关系及其临床意义。方法纳入67例患者和28例对照者,然后根据蒙特利尔认知评估量表(MoCA)测评结果将GMD者分为GMD伴mVCI组(GMD+mVCI组,n=53例)和GMD不伴mVCI组(GMD+NmVCI组,n=14例);测定所有入组病例的空腹血糖(FBG)、糖化血红蛋白、血脂及肝肾功能等生化指标,采用酶联免疫吸附法(ELISA)测定所有受试者的血清APN水平,并结合患者年龄、受教育年限、高血压病程、糖尿病病程等进行临床分析。以MoCA作为因变量、mVCI相关危险因素作为自变量进行多元线性回归(Logistic)分析。结果GMD+mVCI组患者的年龄、收缩压、高血压病程、糖尿病病程均高于GMD+NmVCI组患者和对照组患者(P<0.05);GMD+mVCI组APN水平低于GMD+NmVCI组和对照组(P<0.01);Spearman等级相关分析显示,APN水平与MoCA评分呈正相关(r=0.352,P<0.01);Logistic回归分析显示,年龄、受教育年限、糖尿病病程、高血压病程、APN与认知功能损害具有相关性。结论①血清APN水平与GMD患者mVCI呈负相关;②血清APN水平可作为mVCI的重要检测指标。Objective To investigate the relationship between serum adiponectin(APN)level and mild vascular cognitive impairment(mVCI)with glucose metabolism disorder and its clinical significance.Methods Sixty-seven patients with glucose metabolism disorder(GMD)and 28 controls were enrolled.Then,according to the results of Montreal Cognitive Assessment Scale,the patients with GMD were divided into GMD+mVCI group(n=53 cases)and GMD+NmVCI group(n=14 cases).The biochemical parameters such as fasting blood glucose(FBG),glycosylated hemoglobin,blood lipids and liver and kidney function were measured in all patients.Serum APN levels of all subjects was measured by enzyme-linked immunosorbent assay(ELISA),and clinical analysis was performed in combination with patient age,years of schooling,duration of hypertension,duration of diabetes.MoCA was used as dependent variable and mVCI related risk factors as independent variables for multivariate linear regression(Logistic)analysis.Results The age systolic blood pressure,duration of hypertension and diabetes in GMD+mVCI group were higher than those in GMD+NmVCI group and control group(P<0.05).APN level in GMD+mVCI group was lower than GMD+NmVCI group and control Group(P<0.01);Spearman rank correlation analysis showed that APN level was positively correlated with MoCA score(r=0.352,P<0.01);Logistic regression analysis showed age,years of education,duration of diabetes,course of hypertension and APN is related to cognitive impairment.Conclusions①Serum APN levels are negatively correlated with mVCI in patients with GMD;②Serum APN level can be used as an important indicator of mVCI.
