LEEP刀宫颈锥切术对宫颈上皮内瘤样变患者生育能力的影响研究

摘　　要：目的观察LEEP刀宫颈锥切术对宫颈上皮内瘤样变患者生育能力的影响。方法选取2016年2月至2019年2月本院收治并行LEEP刀宫颈锥切术的50例宫颈上皮内瘤样变患者作为研究组,并对其资料进行回顾分析,同时抽取50例无宫颈锥切手术史的正常妇女作为对照组,观察两组受孕及相关生育情况。结果对照组最终受孕率为90.00%,研究组受孕率为82.00%,比较差异无统计学意义;研究组早产率、胎膜早破发生率和生殖道感染均明显高于对照组(P<0.05),剖宫产率与低出生体质量比例与对照组比较差异无统计学意义;研究组社会因素占比(14.00%)明显高于对照组(4.00%)(P<0.05)。结论行LEEP刀宫颈锥切术并不会对宫颈上皮内瘤样变患者的生育能力产生显著影响,但会造成患者早产、胎膜早破和生殖道感染等不良妊娠结局的概率升高,行LEEP刀宫颈锥切术后的患者选择剖宫产的主要原因为社会因素,因此加强临床与患者的耐心有效沟通,以使患者保持积极的心理状态对治疗具有重要的意义。Objective To observe the effect of LEEP knife and cervical cone on the fertility of patients with intraepithelial neoplasia.Methods 50 cases of cervical intraepithelial neoplasia treated with LEEP knife and cervical cone surgery in our hospital from February 2016 to February 2019 were selected as the research group,and their data were retrospectively analyzed.Meanwhile,50 normal women without cervical cone surgery history during the same period were selected as the control group to observe the pregnancy and related fertility in the two groups.Results The final pregnancy rate was 90.00%in the control group and 82.00%in the study group,with no statistically significant difference.There was no significant difference in cesarean section rate and low birth weight ratio between the control group and the study group.The proportion of social factors in the study group(14.00%)was significantly higher than that in the control group(4.00%)(P<0.05).Conclusion LEEP knife cervical cone cut method is not for cervical intraepithelial neoplasia and fertility of patients have a significant impact,but will cause the premature delivery,premature rupture of membranes and genital tract infection in patients with higher probability of adverse pregnancy outcomes,such as line of LEEP knife cut the cervical cone of the main causes of postoperative patients choose cesarean section for social factors,so to strengthen clinical patience and effective communication with patients,in order to make patients maintain a positive mental state is of great importance to therapy.
