摘  要:目的探讨慢性牙周炎患者多种口腔致病菌与激光辅助龈下刮治和根面平整术(SRP)疗效之间的相关关系。方法纳入2016年6月-2017年6入该院诊治的100例慢性牙周炎患者作为研究对象,均采用激光辅助SRP进行治疗。对治疗前后相关指标进行对比,半定量检测龈下几种主要致病微生物的变化情况。结果治疗前,该组PD、CAL、PLI值、MCP-1、TNF-α和hs-CRP水平分别为(6.49±1.23)mm、(6.94±1.77)mm、(2.66±0.33)、(61.72±10.15)ng/mL、(6.47±0.23)ng/mL、(9.34±1.89)ng/mL,BOP为100.0%,治疗后3个月后,该组上述各指标水平分别为(4.85±0.87)mm、(6.16±1.42)mm、(0.82±0.16)、(34.06±8.13)ng/mL、(5.09±1.07)ng/mL、(8.13±1.54)ng/mL,BOP为47.8%,治疗后各指标均低于治疗前,差异有统计学意义(t=4.292、4.037、4.563、7.526、3.317、3.085,χ^2=12.175,P<0.05)。治疗前后各菌属相对丰度除纤毛菌属均差异无统计学意义(P>0.05)。治疗后P.g、B.f和T.d检出量较治疗前明显减少(P<0.05)。PD改善程度和BOP阳性率分别与3种致病菌检出量存在相关性。结论口腔多种致病微生物与慢性牙周炎患者激光辅助SRP的疗效密切相关,合理控制致病菌可为改善慢性牙周炎治疗效果提供临床指导。Objective To explore the correlation between various oral pathogenic bacteria and the efficacy of laser-assisted subgingival curettage and root planing(SRP)in patients with chronic periodontitis.Methods A total of 100 patients with chronic periodontitis who were admitted to the hospital from June 2016 to June 2017 were included in the study.All patients were treated with laser-assisted SRP.The relevant indexes before and after treatment were compared,and the changes of several major pathogenic microorganisms under the gums were detected semi-quantitatively.Results Before treatment,the levels of PD,CAL,PLI,MCP-1,TNF-αand hs-CRP in this group were(6.49±1.23)mm,(6.94±1.77)mm,(2.66±0.33),(61.72±10.15)ng/mL,(6.47±0.23)ng/mL,(9.34±1.89)ng/mL,BOP was 100.0%,and 3 months after treatment,the levels of the above indicators in this group were(4.85±0.87)mm,(6.16±1.42)mm,(0.82±0.16),(34.06±8.13)ng/mL,(5.09±1.07)ng/mL,(8.13±1.54)ng/mL,BOP was 47.8%,each indexes after treatment were lower than before treatment,and the differences were statistically significant(t=4.292,4.037,4.563,7.526,3.317,3.085,χ^2=12.175,P<0.05).There was no statistically significant difference in the relative abundance of each genus except the ciliates before and after treatment(P>0.05).After treatment,the amount of P.g,B.f and T.d was significantly reduced compared with that before treatment(P<0.05).The degree of improvement in PD and the positive rate of BOP were correlated with the detection of three pathogenic bacteria,respectively.Conclusion Multiple oral pathogenic microorganisms are closely related to the efficacy of laser-assisted SRP in patients with chronic periodontitis.Reasonable control of pathogenic bacteria can provide clinical guidance for improving the treatment of chronic periodontitis.
