摘　　要：目的探讨慢性功能性便秘患者肛门直肠动力的表现,尝试了解慢性功能性便秘患者肛门直肠动力的特征,辅助临床诊断与治疗。方法回顾性研究大连大学附属新华医院盆底中心门诊62例慢性功能性便秘患者一般资料特征、疾病症状特征,通过高分辨率肛门直肠测压比较肛管静息压、最大缩榨压、模拟排便直肠压力、肛管残余压及初始感觉阈值、初始便意阈值、最大容量感觉阈值以及直肠肛门抑制反射(RAIR)等肛门直肠动力学的特征。结果不同年龄初始感觉阈值、初始便意阈值、RAIR反射差异显著(P<0.05);大便干结初始便意阈值、腹部症状初始感觉阈值差异显著(P<0.05)。其他特征未见明显差异(P>0.05);结论慢性功能性便秘患者可能与直肠感觉障碍相关,老年患者、大便干结可能出现直肠低敏,诱发慢性功能性便秘。Objective To investigate the anorectal motility of patients with chronic functional constipation by high-resolution anorectal manometry offering evidence for clinical diagnosis andtreatment.Methods We retrospectively analyzed 62 patients with chronic functional constipation who underwent high-resolution anorectal manometry from clinic of Pelvic Floor Center,the Xinhua Affiliated Hospital of Dalian University.The anorectal manometry parameters includes anal resting pressure,maximum anal squeeze pressure,rectal pressure and anal residual pressure during simulated defecation,rectal sensory thresholds(first sensation,desire to defecate,and maximal tolerable volume),and rectal anal inhibitory reflex(RAIR).Results The elderly group(aged 60 or over)had less first sensation,desire to defecate and RAIR(P<0.05)comparing non-elderly group(aged less than 60).Solid stool(Bristol type 1~2)group had less desire to defecate and abdominal discomfort were associated with more first sensation.There were no obvious differences in other parameters(P>0.05).Conclusion Abnormally increased rectal volume thresholds and decreased rectal sensitivity in old people and solid stool indicating chronic functional constipation might be associated with rectal hyposensitivity.
