摘 要:目的探索重组牛碱性成纤维细胞生长因子(rb-bFGF)滴眼液对白内障超声乳化联合人工晶状体(IOL)植入术后干眼症患者泪液氧化应激指标水平的影响。方法 100例白内障超声乳化联合IOL植入术后干眼症患者为观察对象,按随机数字表法分为对照组和观察组,每组50例。对照组行白内障超声乳化联合IOL植入术+聚乙二醇滴眼液治疗,观察组行rb-bFGF滴眼液+白内障超声乳化联合IOL植入术+聚乙二醇滴眼液治疗。比较两组患者治疗前后干眼症状评分、泪膜破裂时间(BUT)以及泪液氧化应激相关指标水平及不良反应发生情况。结果治疗后,观察组干眼症状评分(5.03±1.73)分低于对照组的(8.28±2.68)分, BUT(9.88±1.35)s长于对照组的(7.62±1.60)s,差异有统计学意义(P<0.05)。治疗后,观察组丙二醛(MDA)为(1.07±0.06)U/L、过氧化脂质(LPO)为(0.94±0.38)μmol/L、一氧化氮(NO)为(20.75±7.74)mmol/L,均低于对照组的(1.97±0.12)U/L、(1.12±0.44)μmol/L、(39.88±6.21)mmol/L,超氧化物歧化酶(SOD)水平为(0.20±0.03)mmol/L,高于对照组的(0.13±0.04)mmol/L,差异有统计学意义(P<0.05)。观察组发生眼内炎症4例(8.00%),对照组发生眼内炎症5例(10.00%),两组患者术后不良反应发生率比较,差异无统计学意义(P>0.05)。结论对白内障超声乳化联合IOL植入术后干眼症患者,采用聚乙二醇滴眼液与rb-bFGF滴眼液联合治疗,可显著改善患者眼部不适,延长BUT,提高患者泪液SOD水平,降低MDA、LPO和NO水平,增强机体抗氧化能力,具有较好的临床效果。Objective To discuss the effect of recombinant bovine basic fibroblast growth factor(rbbFGF) eye drops on tear oxidative stress index of xerophthalmia patients undergoing phacoemulsification combined with intraocular lens(IOL) implantation. Methods A total of 100 xerophthalmia patients undergoing phacoemulsification combined with IOL implantation as observation subjects were divided into control group and observation group by random number table method, with 50 cases in each group. The control group was treated with phacoemulsification combined with IOL implantation + polyethylene glycol eye drops, while the observation group was treated with rb-bFGF eye drops + phacoemulsification combined with IOL implantation + polyethylene glycol eye drops. The dry eye symptom score before and after treatment, tear break-up time(BUT), tear oxidative stress related indexes and occurrence of adverse reactions was compared between the two groups. Results After treatment, the dry eye symptom score(5.03±1.73) points in the observation group was lower than(8.28± 2.68) points, and BUT(9.88±1.35) s was longer than(7.62±1.60) s in the control group. Their difference was statistically significant(P<0.05). After treatment, the malondialdehyde(MDA)(1.07±0.06) U/L, lipid peroxide(LPO)(0.94±0.38) μmol/L and nitric oxide(NO)(20.75±7.74) mmol/L in the observation group was lower than(1.97±0.12) U/L,(1.12±0.44) μmol/L and(39.88±6.21) mmol/L in the control group, and superoxide dismutase(SOD)(0.20±0.03) mmol/L was higher than(0.13±0.04) mmol/L in the control group. Their difference was statistically significant(P<0.05). The observation group had 4 cases(8.00%) of intraocular inflammation, which was 5 cases(10.00%) in the control group, and the difference was not statistically significant(P>0.05).
