摘 要:目的研究KRAB相关蛋白1(KAP1)在食管鳞癌(ESCC)中的表达及意义。方法选取手术切除的原发性ESCC136例、高级别上皮瘤变(HGIN)35例、低级别上皮瘤变(LGIN)29例和距肿瘤边缘5cm以上的正常食管黏膜组织(NEE)37例,采用免疫组化SP法对以上各组织中KAP1蛋白的表达情况进行检测,分析其与ESCC肿瘤发生发展及患者预后的相关性;采用实时荧光定量聚合酶链反应(qRT-PCR)对20例匹配的ESCC和NEE组织中KAP1基因的表达进行检测。结果①KAP1蛋白的表达从NEE(24.3%)、LGIN(31.0%)、HGIN(45.7%)及ESCC(55.9%)呈逐渐增高趋势,差异均具有统计学意义(P=0.002);②KAP1的异常表达与pTNM分期、浸润深度及淋巴结转移相关(P<0.05);③qRT-PCR结果显示,与配对的NEE组织相比,ESCC组织中KAP1的相对表达量明显升高(P<0.01);④136例ESCC患者的5年总体生存率为13.2%。ESCC患者的5年生存率与组织学分级、pTNM、浸润深度、淋巴结转移等具有相关性(P<0.05),KAPl的表达与ESCC患者的预后不相关。结论KAP1的过表达在ESCC的发生、发展和转移中起重要作用,可作为理想的肿瘤标志物辅助ESCC诊断、指导临床治疗。Objective To study the expression and its significance of Kriippel associated box(KRAB)and associated protein 1(KAP1)in esophageal squamous cell carcinoma(ESCC).Methods Immunohistochemistry SP method was used firstly to examine the expression of KAP1 in 136 cases of ESCC,35 cases of high grade intraepithelial neoplasia(HGIN),29 cases of low grade intraepithelial neoplasia(LGIN)and 37 cases of normal esophageal epithelium(NEE).Then the associations of KAP1 expression with clinicopathological data and prognosis were also analyzed.Moreover,qRT-PCR was performed to evaluate KAP1 in a total of 20 matched human ESCC and NEE tissues.Results(1)The expression of KAP1 increased progressively from NEE(24.3%)to LGIN(31.0%),to HGIN(45.7%),and to ESCC(55.9%)(P=0.002).(2)The expression of KAP1 was related to invasive depth,pTNM stage and lymph node metastasis in ESCC(P<0.05).(3)The results of qRT-PCR showed that the relative expression of KAP1 was markedly upregulated in ESCC compared with the NEE tissues(P<0.01).(4)The 5-year OS rate for all 136 ESCC patients was 13.2%.The 5-year OS rate was correlated with histological grade,pTNM,invasion depth,lymph node metastasis.However,the expression of KAP1 was not related to the prognosis of patients with ESCC.Conclusion The abnormal expression of KAP1 may be related to development and metastasis of ESCC,which indicates that it can be used as an ideal marker to assist in the diagnosis of ESCC and to guide the clinical treatment.
