摘　　要：目的通过锥体束CT研究Twin-block矫治器矫治安氏Ⅱ~1错牙合畸形的上、下颌骨的改变,探讨Twin-block矫治器的矫治机制。方法选取31例生长发育期的安氏Ⅱ~1患者,用Twin-block矫治器矫治,矫治前后拍摄锥形束CT测量、分析。结果 31例患者矫治后下颌升支长度(Co-Go)、下颌体长度(Pog-Co)都明显增加,有显著性差异(P<0.01);B-冠状面、B-水平面,距离均增加,有显著性差异(P<0.01),提示下颌向前向下生长。ANB减小,U1/SN减小、L1/MP增加,有显著差异(P<0.01),上颌骨没有显著性变化。结论 Twin-block矫治器能够有效促进下颌骨的生长发育,矫治安氏Ⅱ~1错牙合畸形。Objective To investigate the changes of the maxillary and mandible after treatment of Twin-block appliance in the patients with Angle Class Ⅱ division 1 malocclusion by cone-beam CT and discuss the treatment mechanism of Twin-block appliance. Methods Thirty-one patients with Angle Class II division 1 in the growth period were selected and treated with Twin-block appliance. Cone-beam CT was taken before and after treatment to measure and analyze. Results The lengths of the mandibular ascending branch(Co-Go)and the mandibular body(Pog-Co) in the 31 patients significantly increased and there was significant difference(P<0.01). The distances of B-coronal plane and the B-water plane both increased and were significantly different(P<0.01), suggesting that the growth of the mandible was forward and downward. ANB and U1/SN decreased, while L1/MP increased and there was significant difference(P<0.01). There was no significant change in the maxillary. Conclusion Twin-Block appliance can effectively promote the growth and development of the mandible and correct Angle Class II division 1 malocclusion.
