影响胃癌根治术后患者预后效果的相关因素分析

摘　　要：目的分析影响胃癌根治术后患者预后效果的相关因素。方法选取2011年1月—2012年4月皖南医学院弋矶山医院收治的胃癌患者200例,对患者的临床资料进行统计学分析。结果200例胃癌患者中,根治术后1年生存175例,3年生存134例,5年生存113例,生存率分别为87.5%、67.0%、56.5%。患者不同年龄、手术类型、肿瘤部位、肿瘤直径、术后病理、浸润深度、淋巴结转移、临床分期、神经侵犯、脉管癌栓及术前白蛋白水平的1、3和5年生存率比较,差异有统计学意义(P<0.05),多因素Logistic回归分析显示,胃癌患者根治术后预后效果的影响因素包括肿瘤直径[R^R=1.688(95%CI:1.266,2.250)]、浸润深度[R^R=1.583(95%CI:1.262,1.986)]、淋巴结转移[R^R=1.584(95%CI:1.340,1.872)]。结论影响胃癌根治术后患者预后效果的相关因素包括肿瘤直径、浸润深度、淋巴结转移。Objective To analyze the prognostic factors of patients with gastric cancer after radical resection.Methods The clinical data of 200 cases of patients with gastric cancer from January 2011 to April 2012 in our hospital were analyzed statistically.Results Among the 200 cases of patients with gastric cancer,175 cases survived after 1 years,134 cases survived after 3 years,113 cases survived after 5 years,and the 1,3 and 5 years survival rates were 87.5%,67.0%and 56.5%respectively.The differences of 1 year,3 years and 5 years survival rates between patients with different ages,types of surgery,tumor locations,tumor diameters,postoperative pathology,depth of invasion,lymph node metastasis,TNM stages,perineural invasions,vascular invasions and albumin levels before operation were significant(P<0.05);multivariate logistic regression analysis showed that the prognostic factors of gastric cancer patients after radical operation included tumor diameter[R^R=1.688,(95%CI:1.266,2.250)],depth of invasion[R^R=1.583,(95%CI:1.262,1.986)],lymph node metastasis[R^R=1.584,(95%CI:1.340,1.872)].Conclusion The related factors affecting the prognosis of patients with gastric cancer after radical resection include tumor diameter,depth of invasion and lymph node metastasis.
