摘  要:目的通过监测不同化疗方案患者体内替加氟及5-氟尿嘧啶(5-Fu)血药浓度,结合临床疗效及化疗过程中出现的不良反应,预测不同剂量替加氟对血药浓度及不良反应的影响,为患者早期制定治疗方案,实现个体化用药提供依据。方法纳入郑州人民医院肿瘤科收治的消化道肿瘤患者。按预先制定的化疗方案恒速静脉滴注替加氟240 min后立即采集静脉血5 mL,采用高效液相色谱-紫外法测定替加氟及其代谢物5-Fu的血药浓度。密切观察肿瘤患者化疗过程中及化疗结束后所出现的不良反应,正确评定化疗效果。结果不同剂量替加氟化疗后患者替加氟及代谢物5-Fu血药浓度存在差异不明显,对替加氟向5-Fu的转化率无明显影响。不同剂量替加氟化疗后不良反应无明显变化。结论替加氟剂量的差异对患者5-Fu血药浓度和不良反应有一定影响,监测血药浓度可以用于指导该类药物的个体化使用,以提高临床治疗的针对性和可预见性。Objective To determine the plasma concentrations of tegafur and 5-fluorouracil(5-Fu)combining with the toxicity of drug and their chemotherapy effect,and to predict the effect of different doses of togatur on the plasma concentrations and toxicities of drug,in order to make the chemotherapy regimen for patients and realize the personalized medicine.Methods All the patients with gastraintestinal tumors were from the oncology department of the People’s Hospital of Zhengzhou.The plasma concentrations of tegafur and 5-Fu were detected by using an improved high performance liquid chromatography-ultraviolet detection;the toxicities of drug and their chemotherapy effect should be observed and recorded detailedly during and after chemotherapy.Results The plasma concentrations of tegafur and 5-Fu had the unconspicuous differences among all the patients,the convert rate had no statistical differences.The toxicities of drug and their chemotherapy effect had no unconspicuous changes.Conclusion According to the results,the different doses of togafur can probably influence the 5-Fu concentrations and toxicities.In order to realize the personalized medicine and improve the targetable and predictable of clinical treatment.
