摘 要:目的探讨T2^*校正的双反转角梯度三回波技术对肝脏脂肪含量定量分析的价值.方法前瞻性纳入30例经CT或超声确诊的脂肪肝患者作为研究对象,对所有患者行梯度三回波及1 H-MRS检查,反转角分别设20°及70°各行1次三回波扫描,并计算脂变指数(FI)及肝细胞相对脂肪含量(RLC).以1 H-MRS为参考标准,对梯度双回波、三回波测得的FI进行统计学分析.结果20°双回波、20°三回波、70°双回波、70°三回波测得的FI分别为10.79(8.59,17.14)%,7.62(4.00,13.21)%,14.45(10.75,22.54)%,11.07(6.09,18.74)%,1 H-MRS测得的RLC为12.72(6.91,21.75)%,与70°三回波测得的结果最为接近.双回波、三回波测得的FI与RLC均呈正相关(r 20°双=0.881,P<0.01;r 70°双=0.898,P<0.01;r 20°三=0.946,P<0.01;r 70°三=0.935,P<0.01),且Bland-Altman分析显示各指标与RLC一致性均良好;三回波测得的相关系数均高于双回波;20°三回波测得的相关系数较70°三回波略高,但图像质量较后者有所下降.结论经T2^*校正的梯度三回波技术可在纵向研究中替代1 H-MRS,将在脂肪肝早期筛查、疗效监测中发挥重要作用,但其成像参数(反转角)有待进一步优化.Objective To investigate the value of T2^*-corrected dual flip angles gradient triple echo sequences in the quantitative evaluation of liver fat content.Methods 30 patients with fatty liver diagnosed by CT or ultrasound were enrolled.The patients underwent both gradient triple-echo and^1H MRS examination.The flip angles were set at 20°and 70°for triple-echo respectively.The fat index(FI)by gradient double-echo and triple-echo and the relative lipid content(RLC)by^1H-MRS were measured,and then analyzed statistically.^1H-MRS was regarded as the reference standard.Results The FI measured by 20°double echo,20°triple echo,70°double echo and 70°triple-echo were 10.79(8.59,17.14)%,7.62(4.00,13.21)%,14.45(10.75,22.54)%and 11.07(6.09,18.74)%respectively.RLC measured by H MRS was 12.72(6.91,21.75)%,which was the closest to the results measured by 70°triple echo.The FI measured by double echo and triple echo was positively correlated with RLC(r20°double=0.881,P<0.01;r70°double=0.898,P<0.01;r20°triple=0.946,P<0.01;r70°triple=0.935,P<0.01),and had great consistency using Bland Altman analysis.The correlation coefficent of the triple-echo was higher than that of the double echo.The correlation coefficient of the 20°triple echo was slightly higher than that of the 70°triple-echo,however the quality of the images declined when comparing with the latter.Condusion The T2^*-corrected gradient triple echo sequence can replace^1H MRS in longitudinal studies and will play an important role in early screening and ffect monitoring of the fatty liver,however the imaging parameter(flip angle)of this sequence needs to be further optimized.
