摘 要:目的探讨第三代双源CT双动脉期、双门脉期及其不同组合对肝硬化背景下小肝癌(SHCC)的诊断价值.方法回顾性分析42例SHCC患者的CT资料,比较动脉早期、动脉晚期、门脉早期、门脉晚期对SHCC的检出率及其不同组合对SHCC显示的敏感性.结果动脉早期、动脉晚期、门脉早期、门脉晚期SHCC的检出率分别为73.8%、92.8%、85.7%、95.2%,动脉早期与动脉晚期、动脉早期与门脉晚期SHCC检出率的差异均有统计学意义(P<0.05).动脉晚期+门脉晚期组合(90.5%)对SHCC的检出率高于动脉早期+门脉早期(50.0%)、动脉早期+门脉晚期(73.8%)、动脉晚期+门脉早期(64.3%),差异均有统计学意义(P<0.05).4期联合(95.2%)对SHCC的检出率高于动脉晚期+门脉晚期,但差异无统计学意义(P>0.05).结论动脉晚期、门脉晚期对SHCC均具有较高的检出率,动脉晚期+门脉晚期组合对SHCC的诊断率高、辐射剂量少,可以作为常规诊断模式.Objective To investigate the value of the third dual source CT double arterial phases and double portal venous phases in diagnosis of small hepatocellular carcinoma(SHCC)on the background of liver cirrhosis.Methods CT data of 42 patients with SHCC were analyzed retrospectively.To analyze the detection rate of each phase and the sensitivity of different phase combinations for detecting SHCC.Results The detection ratcs of SHCC by carly arterial phase,late arterial phase,early portal venous phase and late portal venous phase were 73.8%,92.8%,85.7%,95.2%,espectively.Differences in detection rate of SHCC were found statistically between early arterial phase and late arterial phase(P<0.05),early arterial phase and late portal venous phase(P<0.05).The detec-tion rate of SHCC by late arterial phase and late portal venous phase combination(90.5%)was higher than early arterial phase and carly portal venous phase combination(50.0%),early arterial phase and late portal venous phase com bination(73.8%),late arterial phase and early portal venous phase combination(64.3%),the differences were significant(P<0.05).The detection rate of SHCC was the higher in four phases combination(95.2%)comparison with late arterial phase and late portal venous phase com bination,the differences was no significant(P>0.05).Conclusion The late arterial phase and late portal venous phase have higher diagnostie rate in detection of SHCC,and the combination of late arterial phase and late portal venous phase can be used as a routine mode in detection of SHCC with higher diagnostic rate and lower radiation dose.
